¿Por qué se ha recomendado esta operación?

El Dr. Kieran Hirpara, cirujano de cadera y rodilla en el Mater Private Hospital Rockhampton, recomienda la cirugía de revisión de prótesis de cadera cuando una prótesis previamente implantada ha dejado de funcionar correctamente. La cirugía de revisión consiste en reemplazar algunas o todas las piezas de la prótesis original. Por lo general, la indicamos por varias razones: las piezas se han aflojado sin que haya infección, la cadera se ha vuelto inestable y se disloca, o existe una infección alrededor del implante. En su visita, tomamos su historia clínica, examinamos su cadera y, si es necesario, solicitamos estudios de imagen para determinar cuál es el problema. En primer lugar, se intentan opciones no quirúrgicas como modificar las actividades físicas y la fisioterapia, siempre que sea adecuado; la cirugía se realiza cuando estas medidas no logran mejorar suficientemente el cuadro. El objetivo de esta operación es aliviar el dolor, restaurar la función y mantener la estabilidad de su cadera.

Antes de la operación

La planificación comienza con imágenes de su cadera. Se le realizarán radiografías y, a veces, una resonancia magnética (un estudio que muestra los tejidos blandos) o una ecografía. Estas ayudan a identificar el problema y a planificar la operación.

En las semanas previas a la cirugía, prepare algunas cosas prácticas. Deje de comer siete horas antes de la operación. Pedimos que sea siete horas en lugar de seis para poder adelantar su intervención si la lista de quirófanos lo permite. Su cirujano le indicará qué medicamentos habituales debe suspender y cuándo. Lleve una lista por escrito de todos los fármacos que toma. Organice que alguien lo lleve a casa y, el día de la operación, use ropa holgada y cómoda.

Si padece otras enfermedades, es posible que necesite análisis de sangre o una consulta con el anestesista (el médico encargado de administrar la anestesia). La mayoría de las personas no requieren ninguno de estos procedimientos.

El día de la intervención

Llega usted a la unidad de admisiones quirúrgicas del hospital, donde se le registra y prepara para la cirugía. Posteriormente, conoce al anestesista. Esta operación se realiza bajo anestesia general. En ocasiones, se añade un bloqueo nervioso regional para aliviar el dolor postoperatorio; el anestesista hablará con usted al respecto ese mismo día.

Se le lleva al quirófano, donde se realiza la operación. Una vez finalizada, despierta usted en la sala de recuperación, donde las enfermeras lo vigilan mientras la anestesia va desapareciendo. Cuando su estado sea estable, será trasladado a la planta de hospitalización o se le dará el alta para volver a casa, según el tipo de intervención y cómo evolucione su recuperación.

Qué implica la operación

La revisión de un reemplazo de cadera es una operación más larga y compleja que el primer reemplazo de cadera. El cirujano realiza una incisión en el lateral de la cadera, a menudo aprovechando la cicatriz de la operación anterior. La elección del abordaje depende de por qué falló el primer reemplazo, qué partes deben retirarse, si se ha perdido hueso y las cicatrices derivadas de operaciones previas.

Una vez dentro, el cirujano extrae las partes sueltas o desgastadas del implante original. Algunas partes pueden permanecer en su sitio si están firmes y en buena posición. Si se ha perdido hueso alrededor del implante, el cirujano puede reconstruirlo mediante injerto óseo, utilizando hueso de donante o hueso extraído del propio paciente. A continuación, se colocan las nuevas piezas. Según la calidad del hueso, estas pueden fijarse con cemento óseo o presionarse firmemente contra el hueso para que este crezca sobre ellas. Si la cadera ha sufrido dislocaciones, el cirujano verificará que las nuevas piezas queden alineadas de forma que la articulación permanezca estable.

Algunas operaciones son más sencillas que otras. Si solo es necesario cambiar una pieza y el resto está bien fijado, el cirujano podría limitarse a sustituir esa pieza. En cambio, si el hueso alrededor del implante está muy dañado o fracturado, la reparación requerirá una reconstrucción más extensa; a veces se utiliza un fragmento de hueso de donante moldeado para reemplazar una gran porción de hueso perdido.

Al final, el cirujano cierra la incisión con puntos de sutura y la cubre con un apósito. Este se deja puesto durante unos 10 días.

Después de la operación

Despertará en la sala de recuperación, donde las enfermeras lo vigilarán de cerca mientras el efecto de la anestesia desaparece. Se le administrará analgésico antes de salir del quirófano, y las enfermeras se asegurarán de mantener el nivel adecuado para que se sienta cómodo. Su cadera quedará cubierta con un vendaje. Dejamos el vendaje puesto durante unos 10 días; por favor, no lo retire antes de ese plazo a menos que se lo indiquemos. Lo cambiamos o lo retiramos cuando vengamos a verlo. La mayoría de los pacientes se ponen de pie y dan algunos pasos con un andador o muletas al cabo de un día; el fisioterapeuta le enseñará cómo moverse de forma segura. Alguien debe acompañarlo durante las primeras 24 horas después de llegar a casa. Su equipo médico le informará si podrá volver a casa el mismo día o si deberá permanecer una noche en el hospital.

Recuperación

Los primeros días después de una artroplastia de cadera de revisión pueden ser incómodos. Sentirá algo de dolor e hinchazón alrededor de la cadera; la piel cerca de la incisión puede parecer amoratada. Todo esto mejora gradualmente. Tomar los analgésicos según las indicaciones, descansar con la pierna elevada y aplicar compresas de hielo envueltas en una toalla también ayudan. El vendaje se deja puesto durante unos 10 días; lo cambiamos o lo retiramos cuando venga a la consulta.

La mayoría de los pacientes se ponen de pie y dan algunos pasos con andador o muletas al día siguiente. Su fisioterapeuta le enseñará ejercicios sencillos para fortalecer la zona y mantener la cadera en movimiento. Cada día practicará caminar distancias un poco mayores. En casa, deberá evitar doblarse en exceso, girar sobre la pierna operada o cruzar las piernas hasta que su cirujano le indique que es seguro. Algunos movimientos pueden hacer que las nuevas piezas se disloquen mientras los tejidos se curan. Al principio, suele ser más cómodo dormir boca arriba con una almohada entre las rodillas.

A medida que disminuye la hinchazón y recupera la movilidad, pasará del andador a las muletas o un bastón, y luego a caminar sin apoyo. Una vez que su cirujano lo autorice, podrá volver a conducir y retomar sus actividades habituales poco a poco. La recuperación varía según cada persona. Algunas caderas requieren más rehabilitación que otras, por lo que su cronograma podría ser distinto. Su cirujano y fisioterapeuta lo guiarán en cada etapa.

Qué puede salir mal

La mayoría de los pacientes evolucionan bien, pero en ocasiones pueden surgir problemas. Su cirujano y el equipo lo vigilarán de cerca para detectar cualquier anomalía a tiempo.

La infección alrededor de las nuevas prótesis es el problema más grave del que hay que cuidarse. Puede notar un dolor profundo y palpitante que no cede con analgésicos comunes, enrojecimiento que se extiende desde la herida, o secreción de líquido desde la zona quirúrgica. Algunas personas presentan fiebre o malestar general. Si observa cualquiera de estos signos, llame a la clínica ese mismo día. Si se siente muy enfermo, acuda a urgencias. Una infección que afecta a la prótesis es más difícil de tratar si no se detecta a tiempo y puede repercutir negativamente en su salud; por eso nunca debe esperar para buscar ayuda.

En ocasiones, la nueva cadera puede salirse de su cavidad ósea; esto se denomina dislocación. Suele ocurrir de forma repentina, a menudo al realizar algún movimiento que se percibe o escucha. Sentirá un dolor intenso, y la pierna podría parecer más corta o estar girada de forma anormal. Si esto sucede, vaya a urgencias; allí el equipo volverá a colocar la cadera en su sitio. Su cirujano le explicará qué medidas tomar para mantener la estabilidad de la prótesis si el problema se repite.

Con el tiempo, las piezas de la prótesis también pueden separarse del hueso, incluso sin presencia de infección. Este proceso suele ser gradual. Es posible que experimente dolor en la ingle o el muslo meses o años después de una recuperación satisfactoria, especialmente al ponerse de pie o caminar; ese dolor puede recordar al que sentía antes del primer reemplazo. Comente este síntoma en su próxima consulta, en lugar de esperar a que empeore. Los estudios de imagen realizados en revisiones rutinarias suelen revelar lo que está ocurriendo.

Si su prótesis original incluía componentes de metal sobre metal, existe otro riesgo específico: algunas personas reaccionan a diminutas partículas metálicas generadas por el desgaste. Puede notar dolor, hinchazón o un bulto cerca de la cadera, así como ruidos tipo chasquidos o rechinidos. Si cuenta con este tipo de implante, le haremos un seguimiento más estrecho mediante controles y escáneres periódicos; por ello, es fundamental que cumpla con todas las citas programadas.

¿Cuándo deben llamarnos?

Llámenos de inmediato si tiene fiebre, escalofríos o se siente mal en general, o si nota que la herida se pone más roja, hinchada o segrega líquido. Acuda a urgencias si se siente muy mal, si experimenta un dolor intenso y repentino en la cadera, o si la pierna parece más corta o está girada de forma anormal, lo cual podría indicar una dislocación de la cadera. También debe acudir a urgencias si presenta hinchazón o dolor en la pantorrilla, dolor en el pecho o dificultad para respirar, ya que estos síntomas pueden ser señal de un coágulo sanguíneo. Llámenos con urgencia si pierde la sensibilidad en la pierna o no puede moverla.
