Tras una lesión vertebral, el dolor o la sensibilidad pueden aparecer en cualquier parte de la columna, desde la base del cráneo hasta el coxis. Algunas personas lo sienten en un punto concreto; otras, en una zona más amplia de la espalda o el cuello. La mayoría de las lesiones de columna no provocan problemas neurológicos, pero el dolor por sí solo es motivo suficiente para que se examine la columna.

La ubicación de la lesión determina los síntomas que se experimentan. Las fracturas de la zona media y baja de la espalda suelen ocurrir en la unión entre el tórax y la zona lumbar, especialmente tras una caída desde cierta altura. Las lesiones cervicales pueden producirse tras un golpe en la cabeza, y a veces afectan a varios niveles de la columna. Si padece una enfermedad que endurece la columna, como la espondilitis anquilosante, una fractura puede producirse incluso tras un impacto leve y seguir siendo inestable.

El dolor suele intensificarse con el movimiento, al levantar objetos o con cualquier actividad que someta al hueso lesionado a carga. El reposo tiende a aliviarlo. Algunas lesiones duelen más por la noche o al levantarse. Inclinarse hacia adelante, permanecer sentado durante mucho tiempo o levantarse de una silla o cama bajas puede resultar difícil. Girar la cabeza para dar marcha atrás con el coche puede ser doloroso si la lesión está en el cuello. Llevar la compra, subir escaleras o vestirse pueden llevar más tiempo de lo habitual.

Si la médula espinal o los nervios que de ella parten resultan afectados, podría notar debilidad, entumecimiento o cambios en el control de la vejiga o los intestinos. Estos signos son importantes y deben comunicarse de inmediato, pues la función nerviosa puede variar con el tiempo y es necesario examinarla y registrarla.

Su equipo médico también buscará lesiones fuera de la zona dolorida. Se producen fracturas adicionales en otro nivel de la columna en aproximadamente el 10 % de los casos; por ello, las pruebas de imagen abarcan toda la columna y no solo la zona afectada.

¿Qué está ocurriendo realmente?

Su columna vertebral está formada por 33 huesos apilados uno encima del otro. Siete se encuentran en el cuello, doce en el tórax, cinco en la zona lumbar, y el resto se unen entre sí en la base. Cada hueso cuenta con un bloque sólido en la parte frontal que soporta el peso corporal, y un anillo óseo en la parte posterior que protege los nervios que discurren por el centro. Unas fuertes bandas de tejido, llamadas ligamentos, mantienen toda esta estructura unida.

Los huesos y ligamentos funcionan como los tirantes y postes de una tienda de campaña: los postes mantienen la estructura erguida, mientras que los tirantes preservan su forma. Si un poste se rompe o un tirante se rompe, la tienda puede ir inclinándose o plegándose lentamente, aunque a primera vista parezca intacta. Eso es lo que constituye una lesión espinal inestable: un daño que permite que la columna pierda su forma o ejerza presión sobre los nervios que debe proteger.

Algunas zonas soportan mayor carga que otras. El punto de unión entre el tórax y la zona lumbar es donde una sección rígida de la columna, reforzada por las costillas, se encuentra con otra más flexible; por eso absorbe mucha fuerza y se fractura con mayor frecuencia en ese lugar. En la región torácica, además, el canal por donde pasan los nervios es más estrecho, por lo que cualquier hinchazón o fragmentos óseos disponen de menos espacio y tienen más probabilidades de causar problemas neurológicos.

Cuando la médula espinal sufre una lesión, los médicos describen el grado de pérdida de funcionalidad. Una lesión completa implica la ausencia total de sensibilidad o movimiento por debajo del punto afectado. Una lesión incompleta permite que persistan cierta sensibilidad o movimiento por debajo de la lesión; esa función conservada se relaciona con un mejor pronóstico. También es importante la ubicación de la lesión: un daño en el cuello puede afectar a los cuatro miembros, mientras que un daño más abajo afecta únicamente a las piernas.

El dolor y la rigidez que usted siente provienen del propio hueso fracturado y del tejido lesionado. Cualquier debilidad, entumecimiento o alteraciones en la vejiga o el intestino se deben a la presión o al daño que afectan a los nervios.

¿Qué podemos hacer al respecto?

Algunas lesiones requieren algo más que un simple corsé. Si la fractura es inestable, si el hueso se ha desplazado hacia el canal por donde transitan los nervios, o si presenta debilidad, entumecimiento o alteraciones en la función vesical o intestinal, por lo general se recomienda la cirugía. La operación estabiliza la columna para que mantenga su forma y alivie la presión sobre los nervios. En ciertos casos esto se realiza poco después de la lesión, ya que una estabilización temprana permite iniciar la rehabilitación antes. Si usted presenta lesiones en varios niveles o aún está recibiendo tratamiento por otras lesiones graves, podríamos estabilizar la columna de forma preliminar y llevar a cabo la reparación definitiva más adelante, una vez que se haya recuperado del impacto inicial. Si padece una condición como la espondilitis anquilosante que ha rigidizado su columna, la fractura suele ser inestable y requiere cirugía, pues un corsé no basta para mantenerla estable. La decisión quirúrgica es una decisión compartida: le explicaremos en qué consiste, cuál es su objetivo y qué ocurriría si optara por no operarse.

Qué esperar

El pronóstico depende de dos factores: si los nervios se han visto afectados y si la lesión es estable. En la mayoría de las lesiones de columna no se producen problemas nerviosos; en esos casos, la evolución suele ser una curación gradual. Una fractura estable en el tórax o la zona lumbar que se trata sin cirugía tiende a estabilizarse en el transcurso de varios meses mediante el uso de un corsé; a largo plazo, los pacientes reportan menos dolor y una mejor funcionalidad que quienes se someten a cirugía para el mismo tipo de lesión. El hueso se consolida, el dolor disminuye y la persona vuelve a sus actividades habituales de forma progresiva.

Si los nervios sí se ven afectados, el pronóstico depende del grado de función que se mantenga por debajo del nivel de la lesión. Una lesión incompleta, en la que aún se conserva cierta sensibilidad o movilidad, está asociada a un mejor pronóstico. En cambio, una lesión completa, en la que no queda ninguna sensibilidad ni movilidad por debajo de la lesión, presenta un pronóstico mucho más desfavorable. Su equipo médico evaluará y registrará la función nerviosa en varias ocasiones, ya que puede variar con el tiempo; cualquier cambio debe detectarse a tiempo.

Existen otros factores que influyen a largo plazo, incluso cuando la fractura sana bien. Con el paso de los años, la columna puede ir perdiendo su forma tras ciertas fracturas; por ello se realizarán controles para vigilarlo. Si usted tiene 70 años o más, el riesgo de sufrir una nueva fractura en otro nivel de la columna es mayor que en personas más jóvenes; por eso es importante mantener la fortaleza ósea. Si una enfermedad como la espondilitis anquilosante ha provocado rigidez en la columna, normalmente se requiere cirugía y un plan de tratamiento personalizado.

En caso de que haya sufrido otras lesiones graves, la estabilización temprana de la columna dentro de las 24 a 36 horas posteriores a la lesión se asocia a menos complicaciones, estancias hospitalarias más cortas y menor necesidad de ventilación mecánica. Esto se refiere a los primeros días tras un accidente grave; no constituye una garantía de su propia recuperación.

Sea cual sea la lesión, el trauma medular puede afectar su calidad de vida durante años. Un seguimiento riguroso, una actividad física razonable y la notificación inmediata de cualquier debilidad o entumecimiento nuevos le brindarán las mejores posibilidades de una recuperación estable.

Cuándo consultar a un especialista

Acuda de inmediato a urgencias si presenta debilidad reciente, entumecimiento o cambios en el control de la vejiga o los intestinos tras una lesión medular; también si la lesión ocurrió tras una caída desde cierta altura o un golpe en la cabeza. Lo mismo aplica si padece alguna enfermedad que endurece la columna, como la espondilitis anquilosante, ya que las fracturas en esa zona pueden producirse incluso tras un golpe leve y seguir siendo inestables. Solicite una evaluación especializada si el dolor persiste tras una lesión sin mejoría, o si el dolor reaparece acompañado de nueva debilidad o entumecimiento semanas después. Si tiene 70 años o más, mencione cualquier dolor de espalda nuevo tras un tropiezo leve, pues con la edad aumenta el riesgo de sufrir nuevas fracturas en otros niveles.
