Una fractura subtrocantérica es una ruptura del fémur, justo debajo de la cadera. El dolor se localiza en la parte superior del muslo o en la ingle, y suele ser intenso. Ponerse de pie, caminar o apoyar peso en esa pierna lo empeora; en cambio, el reposo con la pierna inmóvil lo alivia.

La fractura ocurre en una zona donde músculos fuertes tiran del hueso fuera de su posición normal. Por eso este tipo de fractura es inestable y difícil de mantener en su sitio durante la cicatrización. Esto también explica por qué el dolor suele intensificarse cuando los músculos se contraen, como al girarse en la cama por la noche o al intentar levantarse de una silla tras haber estado sentado un rato.

Las tareas cotidianas se vuelven difíciles rápidamente: le costará ponerse de pie, subir escaleras o ir al baño sin ayuda. Subir o bajar del coche, o sentarse en el inodoro, implica cargar peso sobre el muslo y suele ser doloroso. Dormir del lado afectado resulta incómodo, y el dolor puede despertarlo.

Algunas fracturas en esta zona aparecen tras el uso prolongado de medicamentos para debilitar los huesos; se denominan fracturas atípicas. Antes de que el hueso se rompa, algunas personas experimentan un dolor sordo o molestia profunda en el muslo durante semanas o meses. Si ha tomado alguno de estos fármacos y presenta nuevo dolor en el muslo, informe a su médico cuanto antes.

La cicatrización en esta zona del fémur puede ser lenta: a veces el hueso tarda más de lo habitual en unirse, e incluso en algunos casos no lo hace sin tratamiento adicional. Esto influye más en su recuperación que el tipo específico de fractura; tanto quienes sufren fracturas atípicas como quienes tienen fracturas comunes suelen presentar resultados similares en general.

Una fractura en este lugar también afecta su bienestar general. Las personas mayores suelen notar un deterioro en su calidad de vida, tanto en los primeros meses como a largo plazo. Esto forma parte normal de esta lesión, y merece ser comentado con su equipo médico durante la recuperación.

¿Qué está ocurriendo realmente?

El fémur soporta todo el peso del cuerpo cuando uno está de pie o camina. La fractura se produce en una zona justo debajo de la cadera, donde el hueso es denso y grueso, y donde las fuerzas que actúan sobre él son las más elevadas de todo el esqueleto. Puede considerarse como el pilar de soporte de la pierna. Cuando se rompe, es imposible apoyarse en esa pierna sin sentir un dolor intenso.

Se trata de hueso cortical denso, la capa externa dura del hueso, no la parte interna esponjosa. Este tipo de hueso cuenta con un suministro sanguíneo limitado; por eso la cicatrización en esta zona suele ser lenta. Además, la fractura a menudo se divide en varios fragmentos en lugar de dejar dos extremos limpios, lo cual complica aún más el proceso de curación.

Alrededor de esta zona del muslo hay músculos fuertes que siguen ejerciendo tensión incluso tras la fractura. Los músculos que flexionan la cadera y mueven la pierna hacia el lado tiran del fragmento superior en una dirección; los músculos que acercan el muslo al cuerpo tiran del fragmento inferior en sentido opuesto, acortándolo. Por eso el hueso no permanece alineado por sí solo, y el dolor aumenta cada vez que esos músculos se contraen.

Algunas fracturas en esta zona se producen tras una caída en personas mayores con huesos debilitados. Otras ocurren tras un traumatismo grave, como un accidente de tráfico, en personas más jóvenes. Un tercer grupo de fracturas se da prácticamente sin caída alguna, en personas que han tomado durante mucho tiempo medicamentos que debilitan los huesos; estas son las fracturas atípicas mencionadas anteriormente.

Dado que el hueso soporta una carga tan elevada y los músculos siguen separando los fragmentos, esta fractura se considera de difícil tratamiento. Existe una probabilidad mayor que en otras fracturas de que el material de fijación fallé o de que el hueso tarde más en unirse. Por ello, el tratamiento suele requerir cirugía para mantener el hueso firmemente en su posición mientras cicatriza.

Qué podemos hacer al respecto

En el caso de esta fractura, la cirugía suele ser el tratamiento principal. La intervención habitual consiste en colocar una barra metálica dentro del canal hueco del fémur, fijándola con tornillos para mantener los fragmentos óseos alineados. A esto se le conoce como clavo intramedular, y es el tratamiento estándar para esta fractura. La barra transmite la carga a través del hueso durante el proceso de curación; esto es crucial, pues las fuerzas que actúan sobre esa zona del muslo son varias veces el peso corporal. En ocasiones menos frecuentes, se emplea una placa y tornillos en la superficie externa del hueso. La opción más adecuada para usted dependerá de la configuración de la fractura y de su estado de salud general; analizaremos este tema conjuntamente para tomar una decisión compartida.

Si el hueso no se une o si la reparación resulta insuficiente, aún es posible recurrir a una cirugía adicional. Esto podría implicar volver a fijar el hueso, utilizar un injerto óseo para favorecer la cicatrización o, en algunos casos, sustituir el fémur por una prótesis articular. Se trata de intervenciones de rescate, cada una con sus propias consideraciones, que abordaremos en su momento.

No se le pedirá que se valga únicamente del reposo y la fisioterapia para recuperarse. La fisioterapia sigue siendo fundamental, pero se aplica después de la cirugía, con el fin de recuperar la fuerza y volver a caminar.

Qué esperar

Con la cirugía, la mayoría de las fracturas en esta zona terminan soldándose. Aquí el hueso se repara lentamente, y eso es normal para esta zona del muslo. Puede esperar una recuperación que se mide en meses en lugar de semanas, ya que el hueso denso y los fuertes músculos que lo rodean hacen que la cicatrización sea más lenta que en muchas otras fracturas.

El pronóstico es bueno cuando el hueso se mantiene firmemente alineado mientras cicatriza. Lograr una buena alineación de los fragmentos óseos es crucial en este caso, y es algo que su cirujano se esfuerza por conseguir durante la operación. Sin tratamiento, la situación es más complicada: los músculos siguen tirando de los fragmentos óseos, por lo que el hueso no se mantiene en su lugar por sí solo. Sin cirugía, no se unirá en una posición útil, y usted no podrá cargar peso sobre esa pierna.

Si el hueso no se une tras la primera operación, no todo está perdido. Una cirugía adicional puede lograr que el hueso se una, y una vez hecho esto, la fractura sanará.
