La infección alrededor de una prótesis de cadera puede manifestarse de distintas formas según cada persona. Algunas personas notan enrojecimiento y calor en la zona de la cadera, hinchazón o fiebre. Otras no presentan ninguno de estos signos; por eso, las infecciones postoperatorias de prótesis de cadera resultan difíciles de detectar.

El dolor es el síntoma más frecuente. Puede estar presente tanto de día como de noche, o intensificarse al mover la cadera, por ejemplo al levantarse de una silla, al caminar o al girarse en la cama. Hay personas que sienten un dolor persistente a pesar de que la prótesis parece funcionar correctamente; este tipo de dolor merece ser tomado muy en serio.

Su historial médico también es relevante. Si tras la operación salió mucho líquido de la herida, si esta se enrojeció e hinchó en varias ocasiones, o si necesitó un tratamiento prolongado con antibióticos, todos estos factores pueden indicar infección. La presencia de un pequeño conducto cerca de la herida que filtra líquido es otro signo que los médicos consideran importante.

Ante la sospecha de infección, el cirujano comenzará tomando un historial clínico detallado y realizando un examen físico, seguido de análisis de sangre para detectar signos de inflamación. Las radiografías pueden mostrar resultados normales o evidenciar cambios alrededor de la prótesis. Con frecuencia, el siguiente paso consiste en extraer una muestra de líquido de la articulación de la cadera mediante una aguja, bajo anestesia local y con ayuda de técnicas de imagen para guiarla. Posteriormente, esa muestra se analiza en busca de infección. Ninguna prueba por sí sola es completamente fiable; por ello, el diagnóstico se establece combinando varios hallazgos.

Un dato importante: cuando se sospecha de una infección tardía, generalmente se posponen los antibióticos hasta concluir todas las pruebas, ya que estos pueden ocultar la presencia de bacterias y dificultar la interpretación de los resultados.

¿Qué está ocurriendo realmente?

El reemplazo de cadera consiste en una esfera metálica que se inserta dentro de una cavidad plástica. Normalmente, ambas partes se deslizan suavemente una contra la otra, como una bisagra bien engrasada. Cuando las bacterias llegan a las superficies del implante, pueden formar una capa pegajosa, similar a un slime, que las protege. A esta capa se le llama biofilm. Una vez que las bacterias quedan dentro de él, resulta muy difícil que el sistema inmunitario o los antibióticos las alcancen. Con frecuencia, las dosis habituales de antibióticos no logran penetrar dicha película, motivo por el cual las infecciones alrededor del implante resultan difíciles de tratar.

El cuerpo reacciona ante las bacterias de la misma forma que ante cualquier invasor: mediante inflamación. Esto provoca hinchazón, calor y dolor alrededor de la cadera. Asimismo, explica por qué el dolor en una cadera infectada suele persistir, incluso cuando el implante permanece firmemente fijado en su lugar. El problema, por lo general, no es que el implante se haya aflojado, sino la infección que lo rodea.

Existen dos patrones principales. La infección temprana aparece poco después de la cirugía, a menudo acompañada de problemas en la herida, como fugas de líquido. La infección tardía puede manifestarse meses o años después; en ocasiones, se debe a que las bacterias viajan a través del torrente sanguíneo desde otras zonas del cuerpo, como la boca, la piel o el tracto urinario. Las infecciones tardías pueden ser difíciles de detectar, siendo el dolor el único indicio.

Las infecciones alrededor del reemplazo de cadera son poco frecuentes: ocurren en aproximadamente el 1% al 2% de los reemplazos de cadera realizados por primera vez. Son más probables tras una cirugía de revisión, es decir, una operación destinada a sustituir o reparar parte de un implante ya existente.

El tratamiento de la infección depende de cuán pronto se detecte, de las bacterias implicadas y de su estado de salud general. Las opciones van desde una lavado mínimamente invasivo con el implante en su sitio, hasta una sola intervención para cambiar las piezas del implante, pasando por dos operaciones separadas en las que se coloca un espaciador antibiótico temporal entre ambas. Su cirujano le explicará cuál es el enfoque más adecuado para su caso y por qué.

Qué podemos hacer al respecto

El Dr. Kieran Hirpara, cirujano de cadera en el Mater Private Hospital Rockhampton, adapta el tratamiento al tipo de infección, a la etapa en que se detecta y a su estado de salud general. Por lo general, los pacientes son derivados a nuestra clínica por su médico de cabecera; si un fisioterapeuta le ha sugerido consultarnos, igualmente necesitará una derivación de su médico de cabecera para poder acceder al reembolso de Medicare. En casos de infección temprana detectada poco después de la cirugía, a veces podemos lavar la articulación de la cadera mediante cirugía artroscópica y dejar el implante en su lugar. Este método funciona en aproximadamente el 65 % al 75 % de los casos. Suele ser más eficaz en reemplazos totales de cadera que en reemplazos parciales realizados por fractura de cadera. Cuando la infección es más profunda o crónica, normalmente recomendamos retirar el implante y colocar uno nuevo. Esto puede hacerse en una o dos intervenciones. En el plan de dos operaciones, extraemos las partes infectadas y colocamos un espaciador temporal que libera antibióticos directamente en la zona. Nuestro objetivo es implantar el nuevo reemplazo al cabo de unos 3 meses, una vez que los análisis de sangre indiquen que la inflamación está remitiendo y las muestras de líquido de la cadera sean negativas. La elección entre una o dos operaciones depende del tipo de bacteria implicada, de su sensibilidad a los antibióticos, de sus demás condiciones de salud y del estado de la piel y los tejidos blandos alrededor de la cadera. Si hay un pequeño conducto cerca de la herida por donde se filtra líquido, o si las bacterias son difíciles de tratar, solemos optar por el plan de dos intervenciones. Los antibióticos forman parte de todos los tratamientos: se administran por vía intravenosa o oral, y a veces se combinan con cemento cargado de antibióticos o injerto óseo durante la colocación del nuevo implante.

En un número reducido de casos complejos, como cuando hay pérdida ósea grave alrededor del implante, puede ser necesario retirar el reemplazo sin colocar uno nuevo para controlar la infección. Esto deja la cadera más corta y débil; casi todos los pacientes afectados necesitan posteriormente un dispositivo de ayuda para caminar. Consideramos esta opción como último recurso, no como una elección habitual.

Independientemente del tratamiento que recomendemos, le explicaremos el razonamiento, qué implica cada opción y cómo afectará a su recuperación. La decisión final es suya, tomada en conjunto con nosotros y su médico de cabecera.

Qué esperar

Con tratamiento, la mayoría de las infecciones alrededor de una prótesis de cadera pueden erradicarse. El desenlace depende de cuán pronto se detecte la infección, de los tipos de bacterias implicados y de su estado de salud general. Las infecciones detectadas poco después de la cirugía suelen tener un pronóstico más favorable, sobre todo si las bacterias responden bien a los antibióticos y usted no presenta otros problemas de salud. Algunos de los factores que influyen en el pronóstico pueden mejorarse antes de iniciar el tratamiento; por eso, su cirujano se encargará de ello en primer lugar, cuando sea posible.

Si no se trata, la infección rara vez desaparece por sí sola. Las bacterias se alojan en una película protectora alrededor del implante, donde los antibióticos y el sistema inmunitario tienen dificultades para actuar. Sin tratamiento, el dolor suele persistir e incluso empeorar, y la infección puede propagarse a través del torrente sanguíneo a otras partes del cuerpo. Por ese motivo, una prótesis de cadera infectada se considera un problema que debe abordarse, no algo que se pueda simplemente esperar que desaparezca.

La recuperación varía según el tipo de tratamiento que reciba. Tras una lavado artroscópico, muchas personas comienzan a moverse rápidamente, aunque es posible que deban seguir tomando antibióticos durante varias semanas. Tras una cirugía para reemplazar las piezas de la prótesis, el proceso es más largo: semanas de cicatrización y luego meses para recuperar la fuerza y la confianza al usar la nueva cadera. Algunas personas necesitan un dispositivo de ayuda para caminar durante un tiempo. También existe la posibilidad de que la infección reaparezca, incluso tras un tratamiento que parecía haber tenido éxito; por eso, su cirujano seguirá controlando sus análisis de sangre y el estado de su cadera en los meses siguientes.

En un pequeño número de casos complejos, el objetivo pasa de curar la infección a controlarla. Los antibióticos a dosis bajas y a largo plazo pueden mantenerla bajo control y ayudar a algunas personas a evitar nuevas intervenciones quirúrgicas. Cuando es necesario retirar la prótesis sin colocar una nueva, la cadera queda más corta y débil; casi todas las personas sometidas a esta operación necesitan posteriormente un dispositivo de ayuda para caminar. Su cirujano solo recomendará este procedimiento cuando se hayan descartado todas las demás opciones.

En resumen: la mayoría de las personas tratadas por una infección en la prótesis de cadera conservan una cadera funcional, pero el camino para lograrlo suele ser largo y rara vez inmediato.

Cuándo consultar a un especialista

Acuda a su médico de cabecera de inmediato si presenta dolor alrededor de la prótesis de cadera que no mejora; en particular, si el dolor persiste aun cuando la cadera parece funcionar bien. Solicite una evaluación especializada si observa enrojecimiento, hinchazón o calor en la zona de la cadera, fiebre, secreción de líquido desde la herida, o un pequeño conducto cerca de la cicatriz que emite líquido. También mencione esto si ha tenido varios episodios de enrojecimiento de la herida, abundante secreción después de la operación, o si ha tomado antibióticos durante un período prolongado. Acuda a urgencias si se siente mal en general, con fiebre y escalofríos, o si el enrojecimiento se extiende; pues una infección alrededor de la prótesis puede propagarse por el torrente sanguíneo y requiere evaluación inmediata.
