Qué está sintiendo

La articulación situada en la base de uno de sus dedos (generalmente el dedo medio) le duele, está un poco hinchada y algo no se mueve como debería. El signo clásico es la sensación de que el tendón en la parte posterior de la articulación “salta” o se desplaza lateralmente al doblar y estirar el dedo. Es posible que note que el dedo se desvía ligeramente hacia el lado del meñique, y que le resulte difícil enderezarlo por sí solo desde una posición doblada; sin embargo, una vez que alguien lo endereza suavemente, el dedo puede mantenerse en esa posición.

Muchas personas experimentan esto por primera vez después de lanzar un puñetazo (contra un saco, una pared o en una pelea), razón por la cual esta lesión se conoce como “dedo de boxeador”. Pero también puede ocurrir con movimientos mucho más leves, como estirar el dedo con fuerza, o puede aparecer gradualmente en personas con artritis inflamatoria. ¿Qué está ocurriendo realmente?

En la parte posterior de cada nudillo, el tendón extensor discurre por el centro del dedo, como un cable dentro de una ranura. Unas finas bandas de tejido llamadas bandas sagitales se sitúan a ambos lados y mantienen dicho tendón centrado sobre la parte superior del nudillo. La banda situada en el lado del pulgar es la más importante.

Cuando esa banda se desgarra o se estira (por un puñetazo, un movimiento brusco y fuerte, o por una inflamación articular crónica), el tendón deja de estar sujeto en su posición y se desliza desde la parte superior del nudillo hacia el espacio entre los huesos, en dirección al lado del meñique. Al quedar el tendón ahora en un lateral en lugar de en la parte superior, no puede estirar el dedo de manera eficaz desde una posición completamente flexionada; además, se mueve de un lado a otro al moverse el dedo. Ese deslizamiento, ese movimiento errático y esa debilidad para estirar el dedo resumen todo el problema.

Qué podemos hacer al respecto

En nuestra clínica, el Dr. Kieran Hirpara aborda esta afección confirmando primero el diagnóstico mediante una evaluación exhaustiva y, si es necesario, estudios por imagen. Por lo general, comenzamos con tratamientos no quirúrgicos para casos crónicos y solo consideramos la cirugía si este enfoque no brinda suficiente alivio.

La buena noticia es que si se detecta a tiempo, con frecuencia se cura sin necesidad de cirugía. Lo esencial es impedir que el tendón se desplace mientras la estructura de soporte se repara por sí sola.

Inmovilización con férula (tratamiento no quirúrgico). El tratamiento más habitual consiste en una férula pequeña y personalizada que impide que la falange lesionada se doble en exceso, permitiendo al mismo tiempo que los demás dedos y sus articulaciones se muevan con normalidad. Una variante muy útil, a veces denominada férula de movimiento relativo o “yugo”, mantiene el dedo afectado ligeramente elevado respecto a los vecinos, lo que mantiene el tendón centrado mientras se sigue utilizando la mano. Por lo general, esta férula se usa durante unas seis a ocho semanas. Si se aplica en las primeras semanas posteriores a la lesión, resulta eficaz para la mayoría de los pacientes.

Cirugía. Si la lesión es de larga data, si la férula no logra estabilizarla o si el tendón se desplaza por completo, la cirugía puede corregir la situación. El cirujano repara directamente la estructura de soporte dañada o la reconstruye empleando una tira de tendón cercana para volver a crear dicha estructura y centrar el tendón sobre la falange. Tras la intervención, el dedo queda protegido con una férula durante un tiempo para favorecer su curación.

Qué esperar

En el caso de lesiones leves tratadas con férula, el pronóstico es bueno: el tendón deja de desplazarse, el dedo vuelve a enderezarse con normalidad y la mayoría de las personas recuperan el uso completo de la mano. No obstante, se requiere paciencia: la férula debe usarse durante varias semanas, y esforzar la mano en exceso demasiado pronto puede provocar que el tendón se desplace nuevamente antes de que la lesión haya sanado.

En lesiones que requieren cirugía (generalmente las más antiguas o graves), la reparación o reconstrucción suele ser eficaz para detener la luxación y restaurar un movimiento fluido; tras ello, se aplica un período de uso de férula y terapia de mano para recuperar la movilidad y la fuerza. Los boxeadores, artesanos marciales y demás atletas suelen poder volver a su deporte, pero la mano necesita tiempo para sanar adecuadamente a fin de evitar que el problema reaparezca.

¿Cuándo consultar a un especialista?

  • Un nudillo que cruje, hace clic o se traba al doblar y estirar el dedo, especialmente después de un puñetazo o un movimiento brusco: es necesario que lo revise un profesional cuanto antes, ya que el uso temprano de una férula marca la diferencia.
  • Dificultad para estirar un dedo por sí solo desde una posición doblada, o que el dedo tienda a desviarse hacia el lado del meñique.
  • Dolor e hinchazón persistentes en un nudillo que no mejoran, o la sensación de que el tendón está “fuera de lugar”.
  • Lesiones en los nudillos en boxeadores o practicantes de artes marciales: estas lesiones suelen ignorarse o sobrellevarse, pero se tratan mejor si se atienden lo antes posible.

En mayor profundidad

Esta sección profundiza más de lo necesario para que usted tome sus propias decisiones terapéuticas. La lesión de la banda sagital merece una lectura adicional, pues se trata del problema extensor que con mayor frecuencia se pasa por alto: el tendón no se ha roto, simplemente se ha desplazado. Además, el uso temprano de la férula es lo que determina si será necesaria o no una intervención quirúrgica.

No hay desgarros; simplemente algo se ha desplazado

El tendón extensor recorre la parte dorsal del dedo, pasando por encima de los nudillos, y queda centrado gracias a una fina cubierta de tejido a cada lado: las bandas sagitales. Cuando la banda radial cede, el tendón se desliza hacia el espacio entre los nudillos.

Las consecuencias derivan directamente de esta mecánica. El dedo puede mantenerse recto una vez que otra persona lo endereza, pues el tendón desplazado aún puede mantener esa posición extendida; sin embargo, no puede enderezarse de forma activa partiendo de una posición doblada, ya que ha perdido su línea de tracción. Esta diferencia entre extensión pasiva y activa es lo que distingue esta lesión de una ruptura tendinosa; por eso, una mano mantenida abierta sobre la mesa puede parecer completamente normal.

Las causas clásicas son una lesión por flexión brusca o forzada de un dedo recto, un golpe directo sobre el nudillo, la llamada “presentación del boxeador”; e incluso, sin ninguna lesión previa, la artritis reumatoide, en la cual dicha cubierta de tejido se debilita con el tiempo y los tendones se desplazan hacia el dedo meñique.

El inmovilizado con férula es eficaz si se inicia a tiempo

La evidencia clínica disponible es cualitativa, no numérica, pero resulta inequívoca. Las lesiones agudas de la banda sagital pueden tratarse con éxito mediante el inmovilizado de el dedo lesionado en posición neutra o de hiperextensión; en cambio, los pacientes con lesiones crónicas o aquellos en los que el tratamiento no quirúrgico no ha surtido efecto podrían beneficiarse de una exploración quirúrgica [1].

La posición en la que se coloca la férula es un aspecto clave. La férula de movimiento relativo o férula en yugo mantiene el dedo lesionado ligeramente más extendido que los dedos vecinos; esto disminuye la tensión sobre la banda en proceso de cicatrización y, al mismo tiempo, permite el uso de la mano. Se trata de una forma de inmovilización relativamente tolerable, lo cual explica por qué la adherencia al tratamiento suele ser buena.

Lo que hace que el momento de iniciar el tratamiento sea crucial es que se trata de una lesión de tejidos blandos que cicatriza mediante formación de tejido cicatricial. Si se inicia el inmovilizado a tiempo, la banda puede recuperar su longitud original, manteniendo el tendón en posición central. Si, en cambio, se deja pasar varios meses, la banda cicatriza en una longitud mayor; el tendón sigue desplazándose fuera de su posición normal, y entonces la única opción viable es la reconstrucción quirúrgica, en lugar de la curación espontánea.

¿Por qué se está revisando la clasificación en lugar de establecerla definitivamente?

La labor actual se ha centrado en perfeccionar la forma de clasificar estas lesiones; se ha propuesto una modificación al sistema de clasificación más utilizado para orientar el tratamiento y estandarizar su descripción [2]. Esto refleja el estado actual de la especialidad: los tratamientos están bastante consensuados, pero aún se está determinando a qué categoría pertenece cada lesión y, por ende, quién requiere cirugía.

En la práctica, esto significa que la solidez de cualquier recomendación clínica depende más de los hallazgos del examen físico que de la categoría asignada. El hecho de que el tendón se subluxe activamente al doblar el dedo, así como el tiempo que esto ocurre, son los factores determinantes para la toma de decisiones.

Pertenece a un grupo de lesiones que suelen confundirse fácilmente

La lesión de la banda sagital es una de las tres lesiones cerradas del mecanismo extensor, clasificadas según el lugar donde se produce la rotura: la lesión en la punta del dedo, la lesión en la articulación media y la lesión de la banda sagital en el nudillo [3]. Cada una requiere una posición de inmovilización distinta; además, la hinchazón del dedo en las primeras fases posteriores a la lesión puede generar una situación poco clara. Dado que el período de tratamiento óptimo para la lesión de la banda sagital se cierra a medida que el tejido cicatriza, es especialmente importante distinguirlas a tiempo, más que en los otros dos casos.
