Con el uso, toda prótesis de cadera sufre cierto desgaste. Las superficies en movimiento se rozan entre sí, y con el tiempo se desprenden pequeñas partículas. Por lo general, la superficie más dura desgasta a la más blanda. En la mayoría de las personas esto ocurre lentamente y no genera ningún problema.

Cuando el desgaste se acumula, las partículas sueltas pueden irritar el tejido circundante a la cadera. Esto provoca dolor en la ingle, que a veces se extiende hasta el muslo. Es posible que note hinchazón cerca de la articulación. El dolor suele intensificarse tras realizar actividad física y disminuye con el reposo. En algunas personas, este dolor incluso interfiere con el sueño nocturno.

El desgaste también puede debilitar la unión entre la prótesis y el hueso. Las tareas cotidianas que someten a la cadera a carga, como levantarse de una silla baja, subir escaleras o caminar hasta el buzón, pueden volverse más difíciles o incómodas. Algunas combinaciones de prótesis más antiguas generaban mayor desgaste que otras. ¿Qué ocurre en realidad?

Una prótesis de cadera funciona como una esfera que encaja en un socket. Cada vez que te mueves, la esfera y el revestimiento del socket se deslizan el uno sobre el otro. Los cirujanos denominan a estas superficies móviles “cojinetes”, de forma similar a como una rueda gira sobre un cojinete. Todos los cojinetes liberan pequeñas partículas al friccionar; la superficie más dura desgasta gradualmente a la más blanda.

Tu cuerpo percibe esas partículas como residuos indeseados. Con el paso de los años, pueden acumularse miles de millones de ellas. Cuando su cantidad es excesiva, el tejido circundante a la cadera se inflama. Esa inflamación puede ir desgastando lentamente el hueso que sostiene la prótesis. Esto se conoce como osteólisis, es decir, pérdida ósea alrededor de la prótesis. Es la causa por la cual el desgaste puede hacer que una cadera antes estable se vuelva inestable.

El tipo de cojinete empleado es relevante. La mayoría de las prótesis modernas utilizan una esfera metálica que se mueve sobre un resistente revestimiento de plástico. Esta combinación cuenta con una larga trayectoria clínica y es la opción habitual hoy en día. En cambio, algunas prótesis más antiguas empleaban metal frotándose contra metal; estas liberaban partículas de cobalto y cromo que irritaban el tejido blando, por lo que este tipo de diseño ha sido prácticamente abandonado.

El desgaste se acelera cuando se producen anomalías entre las superficies. Un arañazo en la esfera metálica puede desgastar el plástico; lo mismo ocurre si queda atrapado en la articulación algún fragmento de cemento u otro trozo de metal. Si las piezas no están alineadas correctamente, las superficies se frotan de manera desigual y el desgaste aumenta. Tu propio patrón de marcha también influye en el proceso.

Los síntomas mencionados anteriormente son consecuencia directa de este proceso: la inflamación del tejido provoca dolor profundo en la ingle y hinchazón; la pérdida ósea hace que la prótesis se afloje, por lo que subir escaleras o sentarse en sillas bajas resulta más difícil. Detectar el desgaste a tiempo es fundamental, pues la pérdida ósea es más fácil de tratar mientras aún no haya avanzado demasiado.

¿Qué podemos hacer al respecto?

El Dr. Kieran Hirpara, cirujano ortopédico en el Mater Private Hospital Rockhampton, adapta el tratamiento según el grado de desgaste que se haya producido en su cadera. Por lo general, los pacientes son derivados a nuestra clínica por su médico de cabecera; si un fisioterapeuta le ha recomendado consultarnos, igualmente necesitará una derivación de su médico de cabecera para poder acceder al reembolso de Medicare. En su primera visita, tomamos su historia clínica, examinamos su cadera y solicitamos radiografías o escáneres cuando resultan necesarios. El desgaste suele producirse de forma gradual, por lo que normalmente comenzamos con las opciones menos invasivas.

El primer paso suele ser la observación y los ajustes. Si su cadera cuenta con un sistema de rodamiento metal-metal, también comprobamos los niveles de metal en su sangre y solicitamos un tipo especial de resonancia magnética capaz de visualizar claramente el área a pesar de la presencia de metal. La cirugía se considera cuando el desgaste ha provocado daños reales o el dolor no cede. Si dichas piezas se han aflojado o la pérdida ósea ha avanzado demasiado, es necesario reemplazar todo el implante. En casos donde el desgaste de la superficie metálica de la cadera ha dañado los tejidos circundantes, retiramos el tejido afectado y reconstruimos la articulación con materiales modernos para el rodamiento.

Qué esperar

En la mayoría de las personas, una prótesis de cadera moderna se desgasta lentamente y no genera problemas durante muchos años. La combinación habitual hoy en día —una esfera metálica que se mueve sobre un inserto de plástico resistente— ha demostrado buenos resultados durante los primeros 10 a 15 años de uso. Las superficies cerámicas, en las que ambas piezas móviles están hechas de un material pulido y muy duro, también han resistido bien el paso del tiempo.

Si el desgaste llega a causar problemas, estos suelen desarrollarse de forma gradual y no de repente. El dolor en la ingle y la hinchazón mencionados anteriormente tienden a empeorar con el transcurso de meses o años, no en cuestión de días. Detectarlo a tiempo marca una gran diferencia. Con controles periódicos, un desgaste en la prótesis a menudo se puede tratar antes de que la pérdida ósea sea significativa; en algunos casos, basta con una intervención menor para reemplazar únicamente el inserto y la esfera.

Dejar una prótesis desgastada sin intervención rara vez da buenos resultados. Las partículas de desgaste siguen acumulándose, la inflamación continúa destruyendo el hueso, y una prótesis que en su momento podría haberse reparado con una operación menor termina requiriendo un reemplazo total. En el caso de las prótesis metal-metal, los riesgos son aún mayores: quienes las llevan tienen mayor probabilidad de necesitar una cirugía de revisión que quienes usan otros tipos de superficies de contacto; además, algunos desarrollan daño en los tejidos blandos que requiere seguimiento riguroso mediante análisis de sangre y estudios por imagen.

Algunas cifras ayudan a comprender la magnitud del problema. Aproximadamente 7 de cada 100 personas con superficies de contacto convencionales reportan dolor en la ingle tras la cirugía de reemplazo de cadera; este porcentaje asciende a 15 de cada 100 en el caso de prótesis metal-metal y a 18 de cada 100 tras una resuperficialización de cadera. En personas de 40 años o menos, la tasa de supervivencia a 7 años tras una resuperficialización de cadera es del 90,5%, cifra inferior a la que logran las superficies de contacto modernas. Las superficies cerámica-cerámica y cerámica-plástico han mostrado resultados muy similares hasta los 15 años de seguimiento.

Ninguna superficie de contacto está exenta de problemas y no se puede garantizar ningún resultado en particular. Lo que sí demuestran los datos es lo siguiente: las superficies de contacto modernas se desgastan menos, generan menos complicaciones y brindan a su prótesis de cadera la mejor posibilidad de durar. Cuándo consultar a un especialista
