La estenosis espinal lumbar consiste en un estrechamiento de los espacios en la parte baja de la columna vertebral. Este estrechamiento ejerce presión sobre los nervios que se extienden hasta las piernas. Los síntomas suelen aparecer de forma gradual, a lo largo de meses o años, en lugar de manifestarse de repente.

El patrón típico se denomina claudicación neurogénica: dolor, entumecimiento o debilidad en las piernas que comienzan al caminar o estar de pie, y mejoran al sentarse o inclinarse hacia adelante. Para sentir alivio pueden ser necesarios hasta 20 minutos de reposo. Muchas personas notan que pueden caminar más distancia cuesta arriba que en terreno llano, ya que inclinarse hacia la pendiente reduce la presión sobre los nervios. Por la misma razón existen algunas soluciones prácticas cotidianas: montar en bicicleta suele ser más fácil que caminar, pues se mantiene una postura inclinada hacia adelante; lo mismo ocurre al empujar un carrito de la compra, que coloca la columna en esa misma posición flexionada.

Es útil conocer la diferencia entre esta condición y los problemas de circulación en las piernas, que pueden producir sensaciones similares. Los problemas circulatorios suelen causar dolor en la parte superior de la pantorrilla, que mejora tras unos 5 minutos de permanecer quieto de pie, y empeora al caminar cuesta arriba o al usar una bicicleta estática. En cambio, el dolor por estenosis mejora al inclinarse hacia adelante o al sentarse.

En la vida diaria, el principal límite suele ser la distancia que se puede recorrer a pie. Es posible que tenga que detenerse a mitad de una compra, buscar un asiento en las tiendas o planificar su día en función de los lugares donde pueda descansar. Permanecer de pie en un mismo sitio, como al hacer cola, puede resultar más difícil que caminar. Los síntomas suelen agravarse tras realizar actividad física o después de estar de pie durante largos periodos.

Algunas personas también experimentan dolor lumbar; este dolor y las limitaciones físicas pueden afectar negativamente su estado emocional, especialmente en mujeres que conviven con un dolor crónico.

Un último dato relevante: muchas personas de su edad presentan signos visibles de estrechamiento en los estudios de imagen sin presentar ningún síntoma. Más del 20 % de las personas mayores de 60 años muestran indicios de estenosis en resonancias magnéticas sin experimentar molestia alguna. Por lo tanto, los resultados de los estudios de imagen y cómo se siente realmente el paciente no siempre coinciden; sus síntomas son lo que debe guiar el tratamiento.

¿Qué está ocurriendo realmente?

Su columna vertebral cuenta con un túnel en su parte central por donde pasan los nervios que llegan a las piernas. En la estenosis, ese túnel se va estrechando gradualmente debido a múltiples factores simultáneos.

El problema suele comenzar con los discos intervertebrales, esos “cojines” situados entre las vértebras que funcionan como amortiguadores. Con el paso del tiempo, estos discos pierden altura, lo que altera la forma en que las pequeñas articulaciones situadas detrás de ellos soportan la carga. Dichas articulaciones y los ligamentos cercanos se vuelven más gruesos y rígidos; asimismo, el ligamento que se encuentra en la parte posterior del túnel, que debería permanecer blando y elástico, pierde flexibilidad con la edad. Todo este tejido adicional ocupa el espacio que antes ocupaban los nervios. Este estrechamiento se produce tan lentamente que los nervios suelen adaptarse a él; por eso muchas personas presentan pocos síntomas nerviosos incluso cuando el cambio está bastante avanzado.

Este estrechamiento es más frecuente en los dos segmentos móviles más inferiores de la columna: L3-L4 y L4-L5. Algunas personas nacen con un túnel más estrecho que otras, y el desgaste por artritis de la columna es la causa más común en general.

La posición de la columna determina cuánto espacio disponen los nervios. Al arquear la espalda hacia atrás, el túnel se estrecha aún más y el ligamento engrosado se pliega hacia su interior; al inclinar el cuerpo hacia adelante, el túnel vuelve a ensancharse. Precisamente por eso la deambulación y el estar de pie provocan los síntomas, mientras que sentarse o inclinarse hacia adelante, como se mencionó anteriormente, los alivia.

La presión e irritación dentro del túnel estrechado, sumadas a una disminución del flujo sanguíneo hacia los nervios, son las causas del dolor y la sensación de pesadez en las piernas. En el punto donde el nervio sale de la columna a través de un canal lateral, la inflamación del propio nervio puede intensificar aún más el dolor.

Si no se interviene, el pronóstico suele ser estable en lugar de empeorar progresivamente. Los síntomas mejoran en entre el 30 % y el 50 % de las personas, mientras que alrededor del 15 % experimentan un empeoramiento con el tiempo.

Qué podemos hacer al respecto

En la mayoría de los casos, iniciamos con tratamiento no quirúrgico. El reposo ayuda, pero únicamente durante un máximo de 2 días. El alivio del dolor mediante antiinflamatorios o paracetamol también puede ser útil. Un programa de ejercicios para estabilizar el tronco —que fortalezca los músculos que sostienen la columna—, combinado con una buena condición física general, constituye el eje del tratamiento conservador. La educación estructurada y la terapia de ejercicios pueden mejorar su bienestar diario. En algunas personas, la fisioterapia y el ejercicio pueden ayudar a controlar los síntomas en las piernas relacionados con la marcha. Por lo general, recomendamos probar este enfoque durante un tiempo razonable, pues no se ha demostrado que esperar antes de la cirugía reduzca su eficacia. La tracción no presenta beneficios comprobados en este contexto, por lo que no la utilizamos.

Los medicamentos pueden ser de ayuda durante los brotes de dolor, aunque su beneficio a largo plazo es limitado. Las inyecciones de cortisona en el espacio circundante a los nervios pueden aliviar el dolor en las piernas por un tiempo determinado, pero no mejoran el funcionamiento diario del paciente; además, ningún estudio ha demostrado su eficacia a largo plazo. La adición de cortisona a una inyección anestésica en dicho espacio tampoco mejoró el dolor ni la funcionalidad a los 12 meses, en comparación con la sola inyección anestésica.

La cirugía se considera cuando el dolor persiste a pesar de haber seguido un tratamiento conservador de forma adecuada, o cuando aparecen debilidad progresiva o problemas de control intestinal y vesical. La intervención principal es la descompresión, que consiste en retirar hueso y tejido engrosado para dar más espacio a los nervios. Algunos pacientes necesitan también una fusión vertebral, mediante la cual se estabiliza la columna cuando es preciso extraer demasiado hueso o cuando ya existe desplazamiento, curvatura o inestabilidad. Los estudios de imagen por sí solos nunca son motivo para operar; lo que realmente importa son sus síntomas y hasta qué punto limitan su vida cotidiana. Analizaremos juntos las opciones disponibles y decidiremos cuál se adapta mejor a su columna y a sus objetivos.

Qué esperar

En la mayoría de las personas, la estenosis espinal se maneja adecuadamente sin cirugía. Se recomienda un tratamiento no quirúrgico inicial para la mayoría de los pacientes; retrasar la cirugía supone poco riesgo para la mayoría de ellos. El tratamiento conservador puede controlar o prevenir el empeoramiento de los síntomas. Si decide no someterse a cirugía, debe esperar que sus síntomas persistan en lugar de desaparecer.

Si opta por la cirugía, el pronóstico suele ser estable más que espectacular. Las personas con estenosis moderada que se sometieron a cirugía de descompresión presentaron menos dolor y discapacidad que quienes recibieron tratamiento no quirúrgico. Este beneficio también se aplica a personas de ochenta años o más. El tratamiento quirúrgico resulta beneficioso a largo plazo; la fusión combinada con descompresión mantuvo una mejora en el dolor y la función durante el seguimiento a medio plazo. No obstante, añadir fusión a la descompresión no mejoró los resultados a los 2 años en pacientes con estenosis aislada; por ello, la fusión se reserva para situaciones específicas.

La recuperación no sigue un patrón lineal, y los resultados varían entre individuos. Algunos factores pueden obstaculizar un mejor desenlace: la entumecimiento crónico en las piernas previo a la cirugía se asocia a peores resultados a los 2 años. Las personas con estenosis grave suelen tener peores resultados que quienes padecen estenosis leve o moderada. Cuantas más cirugías de columna haya tenido previamente, peor será el pronóstico esperado. Asimismo, otras enfermedades concomitantes aumentan el riesgo de complicaciones postoperatorias.

La cirugía conlleva ciertos riesgos que conviene conocer. La descompresión por sí sola puede, en ocasiones, provocar nuevas deformidades o inestabilidad de la columna. La fusión que se extiende hasta el sacro (el hueso situado en la base de la columna) requiere más tiempo, presenta mayor tasa de complicaciones y de reintervenciones, y una mayor probabilidad de que la fusión no se produzca. Algunas personas desarrollan dolor lumbar o en las piernas años después, lo que obliga a nuevas cirugías. Su cirujano le explicará estos riesgos antes de que tome cualquier decisión.

Si también padece artritis de cadera o rodilla y posteriormente se somete a prótesis de cadera o rodilla, la estenosis no influye en la durabilidad de la nueva articulación; no obstante, su funcionalidad y grado de satisfacción podrían ser ligeramente inferiores a los de personas sin estenosis.

¿Cuándo consultar a un especialista?

Acuda a su médico de cabecera si el dolor, entumecimiento o debilidad en las piernas reaparecen al caminar o estar de pie, y mejoran al sentarse o inclinarse hacia adelante. Solicite una evaluación especializada si estos síntomas limitan la distancia que puede recorrer a pie o su vida cotidiana, o si empeoran a pesar del reposo, el ejercicio y el uso de analgésicos. Acuda a urgencias si aparece nueva debilidad en las piernas o los pies, o si surgen problemas recientes en el control intestinal o vesical, ya que requieren evaluación inmediata. También busque ayuda rápidamente si el dolor se vuelve intenso y constante, o si el entumecimiento se extiende rápidamente por las piernas. Lleve consigo una lista de sus síntomas y de los factores que los mejoran o empeoran, pues este patrón suele ser la pista más útil para el diagnóstico.
