¿Por qué se ha recomendado esta operación?

El Dr. Kieran Hirpara, cirujano ortopédico en el Mater Private Hospital Rockhampton, recomienda la artroplastia de rodilla de revisión cuando una prótesis de rodilla previa ha dejado de funcionar adecuadamente. Esta operación consiste en retirar algunas o todas las piezas de la prótesis original e implantar otras nuevas. Normalmente la sugerimos en casos de infección alrededor del implante, aflojamiento del mismo, inestabilidad, rigidez o desgaste del espaciador de plástico. Nuestra evaluación incluye su historial clínico, un examen físico, análisis de sangre si es necesario y radiografías; además comprobamos que el dolor no provenga de otra zona, como la cadera o la espalda. En casos de problemas crónicos, primero solemos intentar tratamientos no quirúrgicos, como modificaciones en las actividades, fisioterapia, uso de férulas o inyecciones; la cirugía se considera únicamente cuando estos tratamientos no logran una mejora suficiente. Antes de la operación

En las semanas previas a la cirugía, planificaremos su operación con todo detalle. Se le realizarán radiografías de la rodilla; a veces también una tomografía computarizada o una resonancia magnética, para poder observar el implante, el hueso que lo rodea y la cantidad de hueso que sea necesario reconstruir. Asimismo, asistirá a una evaluación en la que examinaremos su rodilla, su piel, su estado de salud general, y revisaremos sus historiales quirúrgicos y estudios previos. Si padece otras enfermedades, es posible que necesite análisis de sangre o una consulta con el anestesista antes del día de la operación. En los días previos a la cirugía, le daremos instrucciones claras sobre sus medicamentos, indicándole cuáles debe suspender y cuándo. El día de la operación, no debe ingerir alimentos durante siete horas antes de ella. Solicitamos ese período de siete horas en lugar de las seis habituales para poder adelantar su operación si el programa quirúrgico lo permite. Organice que alguien lo lleve a casa después de la intervención. Use ropa holgada y cómoda, y lleve consigo una lista de los medicamentos que actualmente toma.

El día de la intervención

Ese día, acudirá a la unidad de admisiones quirúrgicas del hospital, donde se le registrará y preparará para la cirugía. Conocerá al anestesista, quien repasará con usted su historial médico y los medicamentos que toma. Esta operación se realiza bajo anestesia general. En ocasiones, se añade un bloqueo nervioso regional para aliviar el dolor postoperatorio; el anestesista hablará de esto con usted ese mismo día. Qué implica la operación

La revisión de una prótesis de rodilla consiste en extraer algunas o todas las piezas de la prótesis original y colocar otras nuevas. El cirujano utiliza el mismo procedimiento general que en la primera operación, normalmente mediante una incisión en la parte frontal de la rodilla, a menudo a lo largo o cerca de la cicatriz previa. El cirujano abre la rodilla y elimina el tejido cicatricial que rodea la articulación, incluidas las bandas adherentes que pueden formarse en la bolsa situada por encima de la rótula y a sus lados. Esta limpieza permite visualizar toda la articulación y apartar la rótula para acceder a las piezas antiguas.

Antes de cerrar la piel, el cirujano lava la articulación y elimina cualquier tejido no sano. Luego se sutura la incisión y se cubre con un vendaje. Si la infección es el motivo de la revisión, la operación puede realizarse en varias fases: las nuevas piezas se colocan en una segunda intervención, una vez que la infección haya desaparecido.

Después de la operación

Despertará en la sala de recuperación y, una vez estabilizado, será trasladado a la planta de hospitalización. Las enfermeras revisarán su rodilla, el nivel de dolor y cómo se siente. Se le planificará un tratamiento para el dolor; si este no se controla adecuadamente, puede informárselo a las enfermeras. Su rodilla quedará cubierta con un vendaje. Dejamos el vendaje puesto durante unos 10 días; por favor, no lo retire antes de ese plazo a menos que se le indique lo contrario. Lo cambiamos o lo retiramos cuando vayamos a verlo. La mayoría de los pacientes se ponen de pie y dan algunos pasos con andador o muletas el primer día; el fisioterapeuta le enseñará cómo hacerlo. Su equipo médico le informará si podrá volver a casa el mismo día o si deberá permanecer una noche en el hospital. Recuperación

En los primeros días y semanas, su rodilla estará adolorida e hinchada. Esto es normal después de una cirugía de esta magnitud. Poco después de la cirugía, podrá levantarse y moverse. La mayoría de los pacientes logran ponerse de pie y dar algunos pasos con andador o muletas ya el primer día. Su fisioterapeuta le guiará en los ejercicios, que se enfocan en doblar y estirar la rodilla, así como en fortalecer el muslo. Continúe realizándolos en casa, en sesiones cortas y frecuentes. Para esta operación no se utiliza férula. Puede caminar por su casa tanto como se sienta capaz, pero evite girar bruscamente, arrodillarse y levantar objetos pesados hasta que su equipo médico lo autorice. Dormir boca arriba con una almohada debajo de la rodilla suele ser la posición más cómoda al principio.

Los hitos de recuperación se manifiestan como cambios concretos, no como fechas fijas. Cuando la hinchazón disminuya, caminar le resultará más fácil. A medida que recupere la movilidad, subir escaleras o entrar y salir del coche le resultará menos complicado. Una vez que su cirujano le dé el visto bueno para conducir, podrá volver a hacerlo. Muchas personas retoman sus actividades habituales tras esta operación, aunque esto suele ocurrir más tarde que después de un primer reemplazo de rodilla. Qué puede salir mal

La mayoría de los pacientes evolucionan bien, pero en ocasiones pueden surgir problemas. Su cirujano y el equipo lo vigilarán de cerca para detectar cualquier anomalía a tiempo.

La infección alrededor de la nueva articulación es el problema al que prestamos mayor atención. Puede provocar que la rodilla se caliente, se hinche y duela, con enrojecimiento que se extiende desde la herida. Algunas personas se sienten malestar general o presentan fiebre. Si nota alguno de estos síntomas, llame de inmediato a la clínica en lugar de esperar a su próxima cita. La infección también puede aparecer meses o años después; por eso, mencione cualquier nuevo dolor o hinchazón en cualquier control, por leve que parezca.

En ocasiones, las piezas nuevas se aflojan sin que haya infección. Generalmente se percibe como un dolor que regresa gradualmente, a menudo peor al estar de pie o caminar, después de un período en el que la rodilla parecía estable. Si nota que su rodilla se siente menos estable de lo habitual, o si el tipo de dolor cambia, coméntele esto en su próxima revisión.

Puede producirse rigidez si la rodilla no logra doblarse y estirarse tanto como se esperaba. Es posible que note dificultad para bajar escaleras cómodamente, o que el movimiento de flexión le resulte tenso y limitado. Su fisioterapeuta supervisará constantemente su amplitud de movimiento. Si el progreso se estanca, informe a su equipo, pues a veces un procedimiento adicional ayuda a recuperar la movilidad.

En raras ocasiones, tras la cirugía puede formarse un coágulo de sangre en la pierna. Se manifiesta como hinchazón y sensibilidad repentinas en la pantorrilla, a veces acompañadas de calor o un dolor intenso. Si nota esto, o si experimenta dificultad para respirar o dolor torácico, acuda a urgencias o llame a una ambulancia.

Algunos problemas de salud aumentan el riesgo de complicaciones tras esta operación, como el hábito de fumar, el sobrepeso y las enfermedades renales. Antes de la cirugía analizaremos sus riesgos personales; además, tomamos medidas durante y después de la intervención para minimizarlos.

¿Cuándo deben llamarnos?

Llámenos de inmediato si tienen fiebre, si la piel alrededor de la herida se vuelve más roja o comienza a exudar líquido, o si su rodilla se calienta y se hincha. Llámenos si sienten un dolor intenso y repentino en la rodilla, o si no pueden sentir ni mover la pierna. Acudan a urgencias o llamen a una ambulancia si la pantorrilla se hincha y se vuelve sensible de forma súbita, o si les cuesta respirar o tienen dolor en el pecho.
