El síndrome de cola de caballo se produce cuando el conjunto de nervios situado en la base de la médula espinal queda comprimido. Estos nervios controlan la vejiga, el intestino y las piernas. Los signos de alerta suelen aparecer todos a la vez, no uno por uno.

Lo más probable es que experimente un fuerte dolor de espalda acompañado de ciática, es decir, dolor que se irradia hacia una o ambas piernas. Sentarse, agacharse o toser suele empeorar el dolor. Este puede ser tan intenso que le resulte difícil caminar, conducir o incluso levantarse del inodoro.

Los cambios más importantes se producen en la vejiga y el intestino. Es posible que le cueste iniciar el flujo de orina o que no consiga vaciar la vejiga por completo. Algunas personas incluso pierden la sensación de necesidad de orinar. También podría notar entumecimiento alrededor del ano o los genitales, así como cambios en el funcionamiento intestinal.

Estos trastornos nerviosos también pueden afectar su vida sexual. Muchas personas siguen teniendo dificultades sexuales mucho después de que se haya aliviado la presión sobre los nervios.

Si no puede orinar en absoluto, o solo consigue expulsar pequeñas cantidades a pesar de sentir la vejiga llena, considere esto una emergencia. Acuda de inmediato a urgencias y comunique que sospecha de síndrome de cola de caballo. Los médicos pueden comprobar cuánta orina queda en la vejiga tras intentar vaciarla; una cantidad residual de 200 ml o más es un signo de alerta muy importante.

También es posible que note que los síntomas empeoran por la noche o al despertar. Permanecer sentado durante mucho tiempo, como en un viaje en coche o una jornada laboral frente al escritorio, puede agravar el dolor de piernas. Acostarse boca arriba con una almohada bajo las rodillas a veces ayuda a aliviarlo.

Esta afección no se puede manejar en casa mediante reposo y analgésicos. Cuando el conjunto de nervios está comprimido, el tratamiento requiere una cirugía urgente para liberar dicha presión. Si presenta alteraciones en la vejiga o entumecimiento, no espere a ver si mejoran por sí solas.

¿Qué está ocurriendo realmente?

La médula espinal desciende a lo largo de la columna vertebral y normalmente termina alrededor del nivel L1-L2, en la zona lumbar. Por debajo de ese punto, los nervios continúan como un conjunto de haces separados que se abren en abanico, semejantes a la cola de un caballo. Los médicos denominan a este conjunto “cola de caballo” (cauda equina). Este conjunto transmite los impulsos nerviosos que controlan la vejiga, el intestino, la función sexual y las piernas.

Cuando algo ejerce presión sobre este conjunto nervioso, dichos impulsos quedan bloqueados. Los nervios que informan a la vejiga de que está llena y los nervios que controlan el músculo encargado de vaciarla provienen del mismo lugar en la columna vertebral. Si la presión afecta a cualquiera de ellos, se puede perder la sensación de necesidad de orinar o la capacidad de vaciar la vejiga adecuadamente. Esta misma presión explica la entumecimiento alrededor del ano y los genitales, así como el dolor y la debilidad en las piernas.

La presión suele deberse a un problema en uno de los discos intervertebrales. Estos discos tienen un centro blando y gelatinoso rodeado por un anillo externo resistente, algo similar a una rosquilla rellena de mermelada. Si el anillo externo se rompe, el centro blando puede desplazarse hacia atrás, hacia el espacio donde se encuentra el conjunto nervioso, comprimiéndolo. La compresión también puede deberse a otras causas, como un estrechamiento del conducto por el que discurren los nervios.

Esta situación no es comparable al dolor de espalda habitual, en el que un músculo dolorido o un disco desgastado provocan molestias sin que haya compresión nerviosa. En este caso, los propios nervios quedan comprimidos y dejan de funcionar correctamente mientras persista la presión. Por eso los cambios en la función de la vejiga e intestino son más relevantes que el dolor, y por qué esta afección requiere una exploración urgente y cirugía para aliviar la presión, en lugar de simplemente reposo y analgésicos.

¿Qué podemos hacer al respecto?

El Dr. Kieran Hirpara, cirujano de columna en el Mater Private Hospital Rockhampton, trata el síndrome de cola de caballo como una emergencia que requiere cirugía, y no como una afección que se pueda tratar en casa primero. Por lo general, los pacientes son derivados a nuestra clínica por su médico de cabecera; si un fisioterapeuta le ha sugerido que nos consulte, igualmente necesitará una derivación de su médico de cabecera para poder acceder al reembolso de Medicare. En la clínica, tomamos su historia clínica, lo examinamos y organizamos las pruebas de imagen necesarias para confirmar el diagnóstico.

La prueba principal es la resonancia magnética, que utiliza imanes para obtener imágenes detalladas de su columna. Esta técnica muestra los discos intervertebrales, los nervios y el estrecho conducto por donde discurren mucho mejor que la tomografía computarizada. El resultado de la resonancia se interpreta junto con sus síntomas, y no de forma aislada, pues una imagen por sí sola no basta para confirmar esta afección. Si no puede realizarse una resonancia magnética, o si las imágenes resultan poco claras, se puede optar por una mielografía por tomografía computarizada: se trata de una tomografía en la que se inyecta un contraste alrededor de los nervios para identificar el punto de presión.

Para esta afección no existe ninguna fase de autogestión ni de fisioterapia. Cuando el haz nervioso queda comprimido, esperar o hacer ejercicio no aliviará la presión, y los cambios en la función vesical e intestinal pueden volverse permanentes. Por ese motivo, no recomendamos ningún período de reposo, uso de analgésicos o fisioterapia como primera medida.

Se recomienda realizar la cirugía de inmediato. La intervención se denomina descompresión, y consiste en retirar lo que ejerce presión sobre los nervios, normalmente la parte de un disco intervertebral desgarrado que ha protrusionado hacia atrás. El objetivo es liberar rápidamente esa presión para que los nervios puedan recuperarse. Una vez que el paciente es evaluado, la cirugía se programa en cuestión de días, no semanas. En un grupo de pacientes operados por compresión nerviosa por un disco, el tiempo de espera promedio fue de 1,1 días en los casos más agudos y de 3,3 días en los demás. Otro grupo esperó entre 12 y 164 horas desde la aparición de los síntomas hasta la cirugía, con un promedio de unas 45 horas. La antigua creencia de que la cirugía debe realizarse en un plazo de seis horas no se sostiene; la rapidez con que el paciente llegó al quirófano no determinó el grado de recuperación nerviosa.

Antes de la operación, conversaremos con usted sobre el procedimiento; podrá plantear dudas y participar en la toma de decisiones. La mayoría de los pacientes sometidos a esta cirugía por compresión nerviosa por un disco recuperaron la fuerza normal en las piernas. Qué esperar

La recuperación del síndrome de cola de caballo varía de una persona a otra. Los nervios pueden recuperarse, aunque no siempre por completo; algunos cambios pueden permanecer de forma permanente. El pronóstico depende de la gravedad de la compresión nerviosa y del tiempo que haya durado dicha presión.

Por lo general, las piernas se recuperan mejor que la vejiga. La mayoría de las personas sometidas a cirugía para aliviar la presión sobre el haz nervioso recuperan la fuerza normal en las piernas. La vejiga suele ser el aspecto más difícil de recuperar: normalmente es la función más afectada antes de la cirugía, y con frecuencia sigue así después. Quienes presentaron síntomas de forma repentina suelen tener más dificultades para recuperar el control de la vejiga que quienes los desarrollaron gradualmente.

Los cambios que pueden persistir no se limitan a la vejiga. Las dificultades sexuales son comunes incluso mucho después de que se haya aliviado la presión, y afectan a muchas personas con el paso de los años. La vejiga, el intestino y otras funciones corporales automáticas también pueden verse afectadas de forma permanente, incluso en personas que partían con buenas condiciones para la recuperación.

Si se trata la afección rápidamente mediante cirugía para eliminar la presión, muchas personas se recuperan bien; los nervios tienen así una verdadera oportunidad de sanar. Si no se trata, la presión persiste y el daño nervioso puede volverse irreversible. Por eso esta afección se considera una emergencia y no algo que se pueda dejar sin intervención.

La recuperación es gradual y puede tardar meses en lugar de días. Algunas funciones se restablecen antes que otras, y es posible que se sigan observando mejoras durante un largo período. Su equipo médico supervisará el funcionamiento de su vejiga, intestino y piernas durante la recuperación, y podrá ofrecerle apoyo ante cualquier secuela que persista, como ayuda para la micción.

Antes y después de la cirugía, hablaremos abiertamente con usted sobre su pronóstico personal, basándonos en sus síntomas y en los resultados de los estudios de imagen. No quedará a la espera de adivinar qué ocurrirá a continuación.

¿Cuándo consultar a un especialista?

Esta afección requiere atención inmediata. Acuda a urgencias si no puede orinar en absoluto, o si solo logra expulsar pequeñas cantidades a pesar de sentir la vejiga llena. Lo mismo aplica si pierde la sensación de necesidad de orinar, o si nota entumecimiento alrededor del ano o los genitales. Estos son signos de que el haz nervioso está siendo comprimido, y requieren evaluación el mismo día, en lugar de una simple consulta con el médico de cabecera.

Solicite una evaluación especializada urgente si padece fuertes dolores de espalda acompañados de ciática, es decir, dolor que se irradia hacia una o ambas piernas, junto con cualquier alteración en el funcionamiento de la vejiga o los intestinos. No espere a ver si los síntomas desaparecen por sí solos. Los médicos pueden comprobar cuánta orina queda en la vejiga tras intentar vaciarla, y solicitar una resonancia de la columna lumbar.
