¿Cómo afecta el alcohol a los huesos?

Los huesos no son una estructura rígida e inerte; son tejido vivo que se descompone y reconstruye constantemente, algo similar a una cuadrilla de mantenimiento vial que nunca deja de reparar la carretera. Unas células especiales llamadas osteoblastos generan nuevo hueso; sin embargo, el consumo excesivo de alcohol debilita estas células, de modo que se produce menos hueso nuevo. Con el paso de los años, esto desequilibra el proceso y genera huesos más delgados y frágiles: la condición conocida como osteoporosis.

El alcohol perjudica a los huesos de varias maneras interrelacionadas:

  • Disminuye directamente la actividad de las células encargadas de formar hueso, por lo que el esqueleto no se renueva como debería.
  • Altera las hormonas que mantienen la fortaleza ósea (por ejemplo, en los hombres puede reducir los niveles de testosterona, esencial para la formación ósea).
  • Interfiere con la forma en que el cuerpo procesa el calcio y la vitamina D (los materiales básicos y el “sistema de transporte” que el hueso necesita), en parte afectando al intestino, al páncreas y al metabolismo de la vitamina D.

Además, el alcohol afecta el equilibrio y el juicio; por eso quienes beben en exceso tienden a sufrir más caídas. La combinación de huesos más débiles y mayor incidencia de caídas es precisamente lo que provoca fracturas de cadera y muñeca.

Lo positivo es que los huesos responden favorablemente cuando se reduce el consumo de alcohol. Diversos estudios indican que parte de la resistencia ósea perdida puede recuperarse tras interrumpir el consumo excesivo. Su esqueleto es más resiliente de lo que podría pensarse.

El alcohol y la cicatrización de fracturas

Si te has fracturado un hueso, su curación es un proceso complejo que requiere la participación de las mismas células encargadas de formar el hueso; el alcohol, sin embargo, inhibe su actividad. Se ha demostrado que el consumo excesivo o el abuso de alcohol retrasa la cicatrización de fracturas y hace que el nuevo hueso (el “callo óseo” que une los extremos fracturados) sea más débil de lo que sería en otras circunstancias. Alcohol, heridas e infecciones

La cicatrización de una herida quirúrgica supone una gran carga para el sistema inmunitario: debe combatir las bacterias, eliminar el tejido dañado y generar piel y tejido subyacente nuevos y sanos. El consumo habitual y excesivo de alcohol debilita el sistema inmunitario y ralentiza este proceso de cicatrización. El resultado práctico, constatado en numerosos estudios quirúrgicos, es una mayor incidencia de infecciones y problemas en la cicatrización en personas que beben en exceso; además, sus heridas se cierran más lentamente y, a veces, las cicatrices resultantes son de peor calidad. Facilitar el trabajo del sistema inmunitario en el período previo a la cirugía es una de las formas más sencillas de reducir dicho riesgo.

En el momento de la cirugía

La cirugía es el momento en que el alcohol tiene mayor relevancia, ya que afecta a varios aspectos simultáneamente:

  • Sangrado. El alcohol interfiere con la coagulación sanguínea normal, por lo que actúa en cierta medida como un anticoagulante. Esto puede provocar un sangrado mayor durante y después de la operación de lo esperado.
  • El anestésico y los analgésicos. El alcohol altera la forma en que el cuerpo procesa estos fármacos, lo que puede hacer que su efecto en el quirófano sea menos predecible; a veces se requieren dosis distintas. Esta interacción es especialmente importante después de la cirugía: mezclar alcohol con opioides (analgésicos fuertes) resulta peligroso, pues aumenta la somnolencia y ralentiza la respiración; asimismo, el alcohol combinado con paracetamol perjudica al hígado. Por eso es mejor evitar el alcohol mientras se toman estos medicamentos.
  • Síndrome de abstinencia alcohólica en el hospital. Este es el aspecto que la mayoría de las personas no prevén. Si el cuerpo se ha acostumbrado al consumo regular de alcohol, interrumpirlo bruscamente (como ocurre al ingresar al hospital) puede desencadenar un síndrome de abstinencia, que abarca desde temblores, sudoración y taquicardia hasta confusión (delirio) y, en ocasiones, convulsiones. Se trata de un riesgo médico real, pero totalmente controlable si el equipo médico está atento a ello. Este es el motivo principal por el cual preguntamos con sinceridad cuánto bebe: no para juzgarlo, sino para garantizar su seguridad y bienestar.

¿Por qué es importante ser honesto con su equipo médico?

Puede resultar incómodo dar una respuesta exacta cuando le preguntan cuánto bebe; muchas personas, instintivamente, redondean la cifra hacia abajo. Le rogamos que resista ese impulso. Su cirujano y su anestesista no están llevando la cuenta; utilizan esa cifra para tomar decisiones reales: cuánto vigilar la posibilidad de hemorragias, cómo planificar su alivio del dolor y si es necesario establecer un plan para prevenir el síndrome de abstinencia, a fin de evitar que se sienta mal en el hospital. Reducción del consumo de alcohol antes y después de la cirugía

Algunos consejos prácticos:

  • Si le es posible, planee un periodo sin alcohol o con bajo consumo en las semanas previas a la cirugía, y manténgalo mientras las heridas y los huesos se recuperan después de la operación.
  • No beba alcohol mientras toma analgésicos opioides o paracetamol: esa combinación es donde realmente radica el peligro.

Cuándo buscar ayuda con mayor prontitud

  • Necesitar beber para sentirse normal o para sobrellevar el día

  • Beber más de lo que pretendía, o tener dificultades para parar una vez que empieza a beber
