Una hernia de disco en la zona lumbar se produce cuando el suave cojín de cartílago situado entre dos vértebras se desplaza y comprime un nervio cercano. Por lo general, todo comienza con dolores intermitentes en la zona lumbar y los glúteos, que pueden durar meses o años antes de que aparezcan síntomas graves. Luego, un movimiento de flexión, como inclinarse hacia adelante para levantar algo, provoca de repente un dolor que se extiende por la pierna. En la mayoría de los pacientes con esta afección, el dolor en la pierna es tan intenso como el dolor de espalda, e incluso peor.

El dolor suele disminuir al descansar, especialmente al acostarse boca arriba con las rodillas apoyadas en almohadas. Sin embargo, tiende a empeorar al hacer esfuerzos, estornudar o toser. Permanecer sentado durante largos periodos, inclinarse hacia adelante o estar de pie durante mucho tiempo también pueden agravarlo. Las tareas cotidianas que requieren flexión y levantamiento, como cargar la lavadora o recoger objetos del suelo, se vuelven difíciles. En los momentos de mayor dolor, algunas personas notan que los músculos de la espalda se tensan y la postura se desvía hacia un lado.

También puede experimentar hormigueo, sensación de pinchazos o entumecimiento en alguna zona de la pierna o el pie. La zona afectada depende de qué nervio esté siendo comprimido. Asimismo, puede aparecer debilidad muscular, que suele ir y venir según la actividad. Es posible que le cueste levantar el pie o los dedos del pie, o que note que una pierna le resulta menos estable que la otra.

Si el disco comprime todos los nervios situados en la base de la columna vertebral, los síntomas pueden incluir entumecimiento en ambas piernas, entumecimiento alrededor del ano y dificultad para controlar la vejiga o los intestinos. Esto requiere atención médica urgente. Si de repente pierde el control de la vejiga o los intestinos, debe buscar ayuda médica de inmediato.

No todo el dolor de espalda y pierna se debe a una hernia de disco. Su cirujano comparará sus síntomas, los resultados de la exploración física y los estudios de imagen antes de recomendar cualquier tratamiento, pues los estudios por sí solos nunca determinan si la cirugía es necesaria.

¿Qué está ocurriendo realmente?

Entre los huesos de la columna lumbar se encuentran unos cojines llamados discos intervertebrales. Cada disco tiene un centro blando y gelatinoso, y un anillo externo resistente. Imagine que el centro actúa como amortiguador y el anillo externo como el sello que lo mantiene en su lugar. El anillo externo está formado por numerosas capas finas enrolladas como las capas de un neumático; cada capa se orienta en una dirección distinta. Esta estructura permite que la columna se doble y gire sin que el disco se desplace.

Con el paso de los años y por el desgaste, el disco pierde agua y su material elástico se vuelve más delgado. Los genes influyen más en este proceso que el levantar pesas o el trabajo físico intenso. Si el anillo externo se rompe, el centro blando puede salir hacia fuera y comprimir algún nervio cercano. La rotura suele producirse en la parte posterior y lateral del anillo, precisamente donde se ubican esos nervios. Los médicos describen este proceso en distintas fases: un abultamiento con el anillo externo aún intacto, una ruptura en la que el centro blando atraviesa el anillo, o un fragmento suelto que se desprende y desciende por el canal espinal.

Esa presión explica los síntomas que acaba de leer. Cada nervio inerva una zona concreta de la piel y un grupo determinado de músculos; por eso, la sensación de hormigueo, entumecimiento o debilidad depende de qué nervio esté siendo comprimido. Además, el material del disco libera sustancias inflamatorias que irritan el nervio; por eso el dolor en la pierna puede ser ardiente o punzante, incluso cuando el disco no toca nada.

La mayoría de estas hernias se producen en la zona baja de la espalda, en los dos discos situados justo por encima del coxis, y son más frecuentes entre los 30 y los 50 años. La buena noticia es que el cuerpo suele resolver este problema por sí solo: las sustancias inflamatorias se disipan y el material desplazado puede reducirse con el tiempo. En la mayoría de las personas que no requieren cirugía, la hernia disminuye espontáneamente.

Qué podemos hacer al respecto

Cuando los comprimidos y la fisioterapia no resultan suficientes, una inyección de esteroides cerca del nervio afectado puede ser de ayuda. La cirugía se considera cuando el dolor persiste a pesar de haber probado adecuadamente las opciones anteriores, o cuando aparecen debilidad u otros problemas nerviosos que empeoran. Las pruebas de imagen por sí solas nunca determinan la necesidad de cirugía. La intervención más habitual es la discectomía, en la cual se extrae mediante una pequeña incisión el fragmento de disco que comprime el nervio. Esperar unas semanas para aplicar tratamientos no quirúrgicos primero no disminuye la eficacia de la cirugía. Antes de tomar cualquier decisión, analizaremos con usted los beneficios y riesgos; además, puede traer a un familiar o amigo a esa conversación.

Qué esperar

En la mayoría de las personas, esta afección mejora con el tiempo. El cuerpo puede reabsorber por sí solo el material discal que se ha desplazado; en la mayoría de quienes no requieren cirugía, esto efectivamente ocurre. Muchas hernias prácticamente no cambian durante cuatro a ocho años. Algunas personas siguen experimentando dolores de espalda intermitentes durante ese período, pero generalmente estos se mantienen bajo control.

Si finalmente necesita cirugía, normalmente se puede esperar entre 6 y 12 semanas mientras se intentan primero los tratamientos menos invasivos. Esperar no disminuye las probabilidades de éxito de la cirugía. Los principales beneficios de la intervención suelen observarse al principio, durante el primer año; con el tiempo, la diferencia entre el tratamiento quirúrgico y el no quirúrgico se reduce. Los buenos resultados tras una cirugía de disco oscilan entre el 46 % y el 97 %, y mucho depende de seleccionar al paciente adecuado para la operación, más que de los detalles técnicos en sí.

La cirugía no es una solución milagrosa; es justo reconocerlo. El problema más frecuente es la aparición de una nueva hernia en el mismo lugar, lo cual ocurre en entre el 3 % y el 7 % de los pacientes. Alrededor del 7 % necesita otra intervención discal dentro de los cinco años posteriores a la primera. Si le sucede a usted, su situación no es peor que la de alguien sometido a su primera operación; aunque es posible que note más dolor de espalda que antes. Una cirugía repetida también puede ser útil, pero las probabilidades de obtener un resultado satisfactorio disminuyen con cada intervención, y el riesgo de complicaciones se triplica o quintuplica respecto a la primera vez.

Toda cirugía de disco conlleva ciertos riesgos: infección, rotura de la membrana que rodea la médula espinal, lesión nerviosa y problemas poco frecuentes como coágulos sanguíneos en los pulmones o compresión de los nervios que controlan la vejiga y el intestino. Su cirujano le explicará cada uno de estos riesgos antes de que usted tome cualquier decisión.

En resumen: la mayoría de las personas mejoran, independientemente de si se someten a cirugía o no. Mantenerse activo, limitar el uso de analgésicos fuertes y darle tiempo al cuerpo para recuperarse son medidas muy útiles.
