La infección periprotésica de la articulación es una infección que se produce alrededor de una prótesis de rodilla. Puede aparecer en cualquier momento después de la operación, a veces meses o incluso años después. La rodilla suele estar dolorida, hinchada y caliente. El dolor, por lo general, no disminuye como cabría esperar en una recuperación normal; además, puede empeorar por la noche o después de estar de pie durante mucho tiempo.

Algunas personas notan que la zona de la incisión está sensible o que de ella se filtra líquido. Los síntomas pueden ser poco específicos. No existe una sola prueba que ofrezca un diagnóstico definitivo. ¿Qué está ocurriendo realmente?

El reemplazo de rodilla consiste en colocar una superficie artificial sobre los extremos del fémur y la tibia. Normalmente, el cuerpo la acepta y la nueva articulación se mueve sin problemas. En el caso de una infección periprotésica, las bacterias se asientan sobre o alrededor de las piezas artificiales y forman una capa denominada biofilm. Una vez que ese escudo se forma, el sistema inmunitario y los antibióticos tienen dificultades para alcanzarlas; por eso la infección puede persistir durante meses y por qué el tratamiento suele requerir tanto cirugía como antibióticos.

La infección irrita el tejido circundante a la rodilla, lo que provoca dolor, hinchazón y calor en esa zona. También puede debilitar la unión entre las piezas artificiales y el hueso, de modo que el implante ya no permanece firme. Esta pérdida de estabilidad es lo que hace que la rodilla duela al soportar peso, y explica la presencia de líquido o secreción en la herida que algunas personas observan.

Esto ocurre en aproximadamente el 1% al 2% de los reemplazos de rodilla. Existen ciertas circunstancias que aumentan el riesgo: si en el pasado ha padecido artritis séptica (una infección dentro de una articulación), es más propenso a desarrollar este problema tras un reemplazo de rodilla que tras un reemplazo de cadera. Asimismo, cualquier complicación en la cicatrización de la herida durante las primeras semanas postoperatorias incrementa la probabilidad de que la infección alcance las zonas profundas alrededor de la nueva articulación.

Existen dos patrones generales: la infección temprana aparece poco después de la operación, cuando las bacterias aún están libres y no han formado su escudo protector; la infección tardía o crónica se desarrolla meses o años después, cuando las bacterias ya están bien protegidas y firmemente adheridas. Esta diferencia es importante, pues las infecciones tempranas a veces se pueden tratar lavando la articulación y manteniendo el implante original. En cambio, las infecciones tardías suelen requerir la extracción del implante y la colocación de uno nuevo en varias fases.

¿Qué podemos hacer al respecto?

El tratamiento depende de cuán pronto aparezca la infección tras la operación. Para infecciones que surgen hasta 12 semanas después de la operación, este método constituye una opción reconocida.

Las infecciones que aparecen más tarde suelen requerir una cirugía mayor: se retira la prótesis infectada, se coloca un espaciador antibiótico en la rodilla y se administran antibióticos durante un período determinado. El espaciador es un dispositivo temporal que mantiene el espacio abierto y libera antibióticos de forma localizada. Una vez controlada la infección, se coloca una nueva prótesis en una segunda operación. Este enfoque en dos etapas es el método habitual para tratar infecciones ya establecidas; en aproximadamente dos tercios de los casos se logra una rodilla libre de infección. Algunas personas con determinados problemas de salud presentan un cuadro más complejo; evaluamos esto junto con usted al planificar el tratamiento.

La elección del tratamiento es una decisión conjunta. Qué esperar

El pronóstico depende en gran medida de cuán pronto se detecte y se trate la infección. Cuando se detecta poco después de la operación, lavar la articulación y conservar el prótesis original suele dar buenos resultados. Cuando se aplica este tratamiento a tiempo, el 76 % de los pacientes mantienen la rodilla libre de infección al cabo de un año. Cuanto antes se actúe, mayores son las posibilidades de éxito; con el paso del tiempo, esas posibilidades disminuyen.

En caso de infecciones detectadas más tarde, normalmente se requiere el enfoque en dos etapas descrito anteriormente. Con este tratamiento, se logra una rodilla libre de infección en aproximadamente dos tercios de los casos. Algunas bacterias son más difíciles de erradicar que otras; además, las infecciones causadas por varios tipos de bacterias suelen tener un curso más complejo. Las personas con otras afecciones médicas también pueden enfrentar mayores dificultades. Si la infección reaparece tras el tratamiento, suele ser necesaria otra cirugía.

Dejar la infección sin tratar no funciona: las bacterias se protegen mediante mecanismos a los que ni el cuerpo ni los antibióticos pueden acceder, por lo que el dolor, la hinchazón y la inestabilidad de la articulación tienden a persistir o empeorar gradualmente. Cuanto más tiempo dure la infección, más difícil resulta tratarla.

La recuperación tras el tratamiento lleva tiempo. Muchas personas observan que su rodilla vuelve a moverse con normalidad una vez eliminada la infección; quienes han sido tratados con éxito suelen comentar que su rodilla se siente mejor que antes de la infección. No obstante, el proceso no siempre es sencillo: algunas personas necesitan más de una operación, y un pequeño número requiere cirugías más complejas si no se logra erradicar la infección por otros medios.

Su equipo quirúrgico le hará un seguimiento riguroso durante los primeros 2 años posteriores al tratamiento, ya que es en este periodo cuando mayor es el riesgo de reaparición de la infección.
