¿Por qué se ha recomendado esta intervención?

El Dr. Kieran Hirpara, cirujano de rodilla en el Mater Private Hospital Rockhampton, comienza por ofrecer las opciones menos invasivas que se adapten a su condición. La cirugía se considera únicamente cuando estos tratamientos no logran mejoría suficiente.

La artroscopia de rodilla es una cirugía mínimamente invasiva. El cirujano observa el interior de la rodilla a través de pequeñas incisiones, utilizando una cámara delgada, y puede tratar cualquier problema que se detecte. Por lo general, la recomendamos para personas que presentan hinchazón, bloqueo articular, sensación de “traba” o inestabilidad de la rodilla, cuando el examen clínico y las radiografías simples indican una causa interna en la articulación. No siempre es necesario realizar una resonancia magnética antes de esta intervención. En casos de artritis degenerativa, podríamos recomendarla después de que 3 meses de terapia de ejercicios no hayan aliviado sus síntomas. Antes de la operación

Antes de la operación, deberá abstenerse de comer y beber durante siete horas. Pedimos siete horas en lugar de seis para poder adelantar su turno si la lista de cirugías avanza antes de lo previsto. Su cirujano le indicará qué medicamentos debe suspender y cuándo; por ello, lleve consigo una lista por escrito de todos los fármacos que toma. Organice que alguien lo lleve a casa después de la intervención, ya que no estará en condiciones de conducir. El día de la operación, vístase con ropa holgada y cómoda. Las imágenes obtenidas mediante radiografías o resonancia magnética ayudan a planificar la cirugía; ya contamos con ellas gracias a su evaluación previa. El día de la intervención

Llega usted a la unidad de admisiones quirúrgicas del hospital, donde se le registra y prepara para el quirófano. Conocerá al anestesista, quien se encargará de administrarle la anestesia y el alivio del dolor durante la operación. Esta intervención se realiza bajo anestesia general. En ocasiones, se añade un bloqueo nervioso regional para controlar el dolor postoperatorio; el anestesista hablará con usted al respecto ese mismo día. A continuación, se le lleva al quirófano, donde se lleva a cabo la operación.

Una vez finalizada la operación, despierta usted en la sala de recuperación. Allí, las enfermeras lo vigilan mientras la anestesia va desapareciendo. Cuando se haya estabilizado, será trasladado a una sala de hospitalización o podrá volver a casa, según el tipo de intervención y su recuperación. Muchas personas regresan a casa el mismo día.

¿En qué consiste la operación?

La artroscopia de rodilla es una cirugía mínimamente invasiva. El cirujano realiza dos o tres incisiones pequeñas, de aproximadamente 1 cm cada una, alrededor de la parte frontal de la rodilla. Por una de ellas se introduce una cámara delgada que muestra el interior de la articulación en una pantalla; las demás permiten introducir instrumentos finos. En algunos casos, los problemas se localizan en la parte posterior de la rodilla, por lo que puede ser necesaria otra pequeña incisión para acceder a ellos.

Lo que se realiza en el interior depende de la causa de sus síntomas. Se pueden extraer fragmentos sueltos de cartílago u hueso que provocan bloqueos o atascos en la rodilla. Un menisco desgarrado, esa almohadilla elástica que amortigua la articulación, puede ser recortado hasta obtener un borde liso y estable. El cartílago desgastado o deshilachado en las superficies articulares puede ser pulido con un pequeño instrumento de raspado. Si el daño cartilaginoso llega hasta el hueso, el cirujano puede realizar diminutos orificios en dicha superficie para estimular el sangrado; la sangre forma un coágulo que, con el tiempo, se transforma en un nuevo tejido más liso, proceso que dura unas 8 semanas. Durante la operación también se irriga la rodilla con líquido, lo que elimina restos sueltos y tejido irritado.

Las incisiones son muy pequeñas, por lo que se cierran con puntos simples o tiras adhesivas y se cubren con un vendaje. Al ser tan diminutas, no generan heridas de gran tamaño que requieran curación. La mayoría de los pacientes no presentan restricciones de actividad relacionadas con la rodilla a las 4 semanas de la artroscopia; además, para muchas personas la intervención se realiza como cirugía ambulatoria. Su cirujano le informará sobre lo que se encontró en el interior de la rodilla y qué tratamiento se aplicó, lo cual le ayudará a planificar su recuperación.

Después de la operación

Despertará en la sala de recuperación, donde las enfermeras lo vigilarán mientras el efecto de la anestesia desaparece. Sobre las pequeñas incisiones de su rodilla habrá un vendaje; lo dejaremos puesto durante unos 10 días. Por favor, no lo retire antes de ese plazo a menos que se lo indiquemos. Lo cambiaremos o lo quitaremos cuando vengamos a verlo. Antes de que se vaya, le administraremos analgésicos; su equipo le explicará cuáles tomar y cuándo. La mayoría de los pacientes caminan el mismo día, con la ayuda del personal de enfermería. Alguien debe quedarse con usted durante las primeras 24 horas después de regresar a casa. Recuperación

Durante los primeros días, su rodilla estará adolorida e hinchada. La hinchazón suele alcanzar su punto máximo al principio y luego disminuye en las semanas siguientes. Mantener la rodilla elevada mientras está sentado o acostado ayuda, y las compresas de hielo alivian la molestia. Tome los analgésicos que su equipo médico le recetó antes de salir del hospital; el alivio del dolor administrado a tiempo es más eficaz que esperar a que el dolor empeore.

La mayoría de las personas caminan el mismo día de la operación. Su fisioterapeuta le guiará en ejercicios para recuperar el movimiento y fortalecer los músculos del muslo que sostienen la rodilla. Realizar estos ejercicios de forma ligera y frecuente es más importante que hacerlos con gran esfuerzo. Puede moverse por la casa según se sienta capaz; muchas personas logran realizar tareas cotidianas sencillas en pocos días. Evite levantar objetos pesados, agacharse y practicar deportes hasta que su cirujano y fisioterapeuta se lo autoricen.

Las pequeñas incisiones cicatrizan rápidamente; el vendaje permanece puesto unos 10 días, hasta que lo revisemos. A medida que la hinchazón disminuye, doblar y estirar la rodilla resulta más fácil. Una vez que pueda caminar con estabilidad y sienta que su rodilla está bajo control, podrá retomar sus actividades normales poco a poco. La recuperación varía de una persona a otra. Algunas rodillas se estabilizan en un par de semanas; otras necesitan más tiempo. Su cirujano y fisioterapeuta le indicarán el cronograma adecuado en cada revisión.

Qué puede salir mal

La mayoría de los pacientes evolucionan bien, pero en ocasiones pueden surgir problemas. Su cirujano y el equipo lo vigilarán de cerca para detectar cualquier anomalía a tiempo.

La infección es el problema más frecuente tras esta operación. La piel alrededor de las pequeñas incisiones puede volverse roja, caliente y sensible; además, el enrojecimiento puede extenderse desde la herida. Es posible que note secreción o pus saliendo de una incisión, o que tenga fiebre. Los analgésicos comunes podrían no aliviar un dolor profundo y palpitante. Si observa alguno de estos signos, comuníquese con la clínica en lugar de esperar a su próxima consulta.

Otro problema que vigilamos es la formación de coágulos sanguíneos en las piernas. Generalmente se manifiesta como hinchazón y sensibilidad repentinas en la pantorrilla, a menudo más en una pierna que en la otra. La pantorrilla puede sentirse caliente o lucir más roja de lo habitual. Algunos coágulos no provocan síntomas en absoluto; por eso le explicamos este riesgo antes de la operación y tomamos medidas para reducirlo. Si nota una pantorrilla hinchada y dolorosa, llame a la clínica ese mismo día. Si experimenta dificultad para respirar o dolor torácico, acuda de inmediato al servicio de urgencias.

En ocasiones, durante la cirugía artroscópica pueden irritarse los nervios cercanos a la rodilla. Esto puede provocar entumecimiento, hormigueo o una zona de piel que pierde sensibilidad. La mayoría de estos síntomas desaparecen, pero comunique cualquier entumecimiento u hormigueo en su siguiente revisión para que podamos vigilarlo.

Después de la cirugía, la rodilla a veces se vuelve más rígida de lo esperado. Aun cuando la hinchazón disminuye, doblarla o estirarla puede seguir siendo difícil; además, podría sentir un “trabante” o un ruido de roce al moverla. Continúe realizando los ejercicios de fisioterapia y avise a su fisioterapeuta o cirujano si la rodilla no gana movilidad.

Los problemas en la herida, como una incisión que sigue supurando o no cicatriza, son poco frecuentes pero es importante detectarlos a tiempo. Indique cualquier anomalía en las incisiones cuando revisemos el vendaje, alrededor del décimo día.

¿Cuándo deben llamarnos?

La mayoría de los problemas después de esta operación son poco frecuentes; sin embargo, algunos requieren atención inmediata. Llámenos si presenta fiebre, aumento de enrojecimiento o secreción en las incisiones, o dolor intenso y repentino en la rodilla. Llámenos el mismo día si la pantorrilla se hincha y le duele. Acuda a urgencias si le cuesta respirar o siente dolor en el pecho. Acuda a urgencias si pierde la sensibilidad en la pierna o no puede moverla.
