La deformidad espinal en adultos significa que su columna vertebral ha desarrollado una curvatura que no debería tener. ¿Qué está ocurriendo realmente?

Su columna vertebral está formada por una serie de vértebras, separadas entre sí por discos que actúan como amortiguadores. Los discos y las pequeñas articulaciones situadas detrás de ellos comparten la carga que soporta el cuerpo. Cuando todo está alineado correctamente, dicha carga se distribuye de forma uniforme a lo largo de la columna hasta llegar a las caderas.

En la deformidad espinal del adulto, esta alineación se altera. En lugar de una columna recta con curvas naturales suaves, la columna presenta una curvatura lateral, una joroba hacia adelante, o ambas. Esta curvatura se mide mediante un ángulo en una radiografía; si dicho ángulo supera los 10 grados en un adulto ya desarrollado, se considera una deformidad espinal.

Existen dos causas principales. En algunas personas, la curvatura comenzó años atrás, a menudo en la adolescencia, y permaneció asintomática hasta la edad adulta. En otras personas, la curvatura es reciente y se desarrolla a medida que los discos y articulaciones de la zona lumbar se desgastan con el tiempo. Las articulaciones desgastadas ya no pueden mantener la estabilidad de la columna, por lo que un segmento vertebral se desliza y se inclina lentamente respecto al siguiente, generando así la curvatura.

El cuerpo intenta compensar esta situación. Para mantener los ojos nivelados y el equilibrio, las caderas y la zona lumbar modifican su postura; sin embargo, estos ajustes imponen una carga adicional sobre articulaciones que ya se encuentran desgastadas.

Qué podemos hacer al respecto

Existen varios tipos de operaciones, y elegimos la más adecuada para su columna. Algunas personas solo necesitan aliviar la presión sobre los nervios, procedimiento conocido como descompresión. Otras requieren descompresión sumada a una fusión vertebral limitada a unos pocos niveles. Las curvaturas más pronunciadas o rígidas exigen una fusión más extensa que enderece la columna y la mantenga en esa posición mientras los huesos se unen. En casos de curvaturas muy rígidas, podríamos cortar y remodelar parte de una vértebra para restablecer el equilibrio. La cirugía para deformidades de la columna en adultos es una intervención mayor; queremos que cuente con toda la información necesaria antes de tomar una decisión.
