¿Por qué se ha recomendado esta operación?

El Dr. Kieran Hirpara, cirujano de cadera de nuestra clínica, realiza artroscopia de cadera, una intervención mínimamente invasiva que emplea pequeñas incisiones y una cámara para tratar problemas dentro de la articulación de la cadera. Por lo general, los pacientes son derivados a nuestra clínica por su médico de cabecera; si un fisioterapeuta le ha sugerido que nos consulte, igualmente necesitará una derivación de su médico de cabecera para poder acceder al reembolso de Medicare. Evaluamos su historial clínico, examinamos su cadera y, si es necesario, solicitamos estudios de imagen como radiografías, resonancia magnética o tomografía computarizada. La razón principal por la que recomendamos esta operación es el desgaste del labrum y del cartílago provocado por la presencia de hueso adicional en la parte anterior de la cadera, una afección conocida como conflicto femoroacetabular o FAI. Esta intervención también puede ser útil en casos de desgarros del labrum, fragmentos sueltos de cartílago y algunas causas de inestabilidad de la cadera.

Por lo general, primero intentamos tratamientos no quirúrgicos, como modificar sus actividades y trabajar con un fisioterapeuta. La cirugía se realiza únicamente cuando esas medidas no logran mejorar suficientemente su estado. El objetivo es aliviar el dolor en la ingle y ayudarle a volver a caminar, sentarse, correr y girar sin sentir molestias. Cuando existen motivos adecuados para la cirugía, pacientes de cualquier edad pueden mejorar; además, la operación busca frenar la progresión de la artritis.

Antes de la operación

En las semanas previas a la cirugía, confirmamos el plan mediante estudios de imagen como radiografías, resonancia magnética o ecografía, y le indicaremos qué exámenes son necesarios. El día de la operación, no debe ingerir alimentos ni bebidas durante siete horas antes. Pedimos este tiempo para poder adelantar su intervención si el programa quirúrgico lo permite. Algunos medicamentos deben suspenderse antes de la cirugía; su cirujano le dará instrucciones precisas al respecto. Lleve una lista por escrito de todos los fármacos que toma, incluidos los suplementos. Organice que alguien lo lleve a casa después de la operación, ya que no podrá conducir usted mismo. Use ropa holgada y cómoda, fácil de poner y quitar. Si padece otras enfermedades, es posible que también necesite análisis de sangre o una consulta con el anestesista antes del día de la intervención.

El día de la intervención

Llegará a la unidad de admisiones quirúrgicas del hospital, donde se le registrará y preparará para la cirugía. Posteriormente, conocerá al anestesista, el médico encargado de administrar la anestesia y de cuidar de usted durante la operación. Esta intervención se realiza bajo anestesia general. En ocasiones, se añade un bloqueo nervioso regional para aliviar el dolor postoperatorio; el anestesista hablará con usted al respecto ese mismo día. A continuación, será conducido al quirófano, donde se llevará a cabo la operación.

Una vez finalizada la intervención, despertará en la sala de recuperación. Allí, las enfermeras lo vigilarán mientras la anestesia va desapareciendo. Cuando su estado sea estable, será trasladado a una sala de hospitalización o podrá volver a casa ese mismo día, según el tipo de procedimiento y cómo evoluione su recuperación.

¿En qué consiste la operación?

La artroscopia de cadera es una intervención mínimamente invasiva. El cirujano realiza dos o tres incisiones pequeñas, de aproximadamente 1 cm de longitud, alrededor de la cadera. A través de estas incisiones introduce una cámara delgada y pequeños instrumentos dentro de la articulación.

Antes de introducir los instrumentos, se aplica una tracción suave a la pierna; esto consiste en tirar de ella de forma constante para abrir la articulación y crear un pequeño espacio de trabajo entre la cabeza femoral y la cavidad acetabular. La guía por rayos X ayuda al cirujano a localizar los puntos adecuados para las incisiones y a trabajar de manera segura dentro de la cadera.

Una vez dentro, el cirujano trata el problema detectado en las imágenes diagnósticas. En el caso de la FAI, esto implica resecar el exceso de hueso en la parte frontal de la cadera que comprime el labrum y el cartílago. Un labrum desgarrado puede ser alisado o reparado; los fragmentos sueltos de cartílago pueden ser eliminados. El objetivo es evitar que los huesos se atasquen durante el movimiento y proteger la superficie articular.

Al final de la operación, el cirujano cierra la capa de tejido que rodea la articulación, llamada cápsula. Suturar esta capa ayuda a mantener la estabilidad de la cadera durante la cicatrización. Posteriormente, las pequeñas incisiones cutáneas se cierran con puntos de sutura y se cubren con un apósito.

Esta operación trata problemas en distintas zonas de la cadera: el compartimento central (la articulación principal de tipo esfera y cavidad), el compartimento periférico (justo fuera del revestimiento articular) y el compartimento peritrocantérico (la zona lateral de la cadera donde se encuentran algunos tendones). El cirujano le explicará qué áreas están implicadas en su caso.

Después de la operación

Despertará en la sala de recuperación y, una vez que se estabilice, será trasladado a la planta de hospitalización. Sentirá dolor y rigidez en la cadera; el personal de enfermería le administrará analgésicos para mantenerlo cómodo. Durante las primeras 24 horas después de volver a casa, debería haber alguien con usted. Tendrá un vendaje sobre las pequeñas incisiones; lo dejamos puesto durante unos 10 días; por favor, no lo retire antes de ese plazo a menos que se lo indiquemos. Lo cambiamos o lo retiramos cuando vengamos a verle. La mayoría de los pacientes comienzan a caminar con muletas poco después de la cirugía; su fisioterapeuta le guiará en sus primeros pasos. Su equipo médico le informará si podrá volver a casa el mismo día o si deberá permanecer una noche en el hospital.

Recuperación

Durante los primeros días, sentirá dolor y rigidez en la cadera. Es normal que se produzca hinchazón alrededor de la cadera y el muslo; esta disminuirá con el paso de los días. El descanso, el hielo y los analgésicos recetados le ayudarán a aliviar la molestia. Mantenga el vendaje puesto durante unos 10 días, tal como se indicó anteriormente.

En los primeros días, caminará con muletas; su fisioterapeuta le guiará en sus primeros pasos. Poco después de la cirugía, comenzará a realizar ejercicios suaves. Estos pueden hacerse en casa o en sesiones formales de fisioterapia; ambas opciones son igualmente eficaces, así que puede elegir la que prefiera. El programa se adaptará a sus objetivos personales y a sus necesidades diarias; su fisioterapeuta supervisará de cerca su progreso para evitar irritar los tejidos blandos de la zona de la cadera.

A medida que el dolor disminuya, notará que vuelve a caminar con normalidad. La longitud de sus pasos, su velocidad al caminar y el número habitual de pasos diarios se recuperarán durante la fase inicial de rehabilitación. Una vez que su cirujano le autorice a conducir, haya dejado de tomar analgésicos fuertes y pueda reaccionar adecuadamente en una frenada de emergencia, podrá volver a conducir. La recuperación varía de una persona a otra. Su cronograma personal puede ser distinto; su cirujano y fisioterapeuta le guiarán en cada etapa.

Qué puede salir mal

La mayoría de los pacientes evolucionan bien, pero en ocasiones pueden surgir problemas. Su cirujano y el equipo lo vigilarán de cerca para detectar cualquier anomalía a tiempo.

Algunas personas notan entumecimiento, hormigueo o cambios en la sensibilidad alrededor de la cadera, la ingle o el muslo después de la cirugía. Esto se denomina neurapraxia, una irritación temporal de un nervio. Por lo general, desaparece por sí sola; sin embargo, si no es así, informe a su cirujano en la próxima revisión. Cualquier entumecimiento o debilidad que aparezca de forma repentina y no ceda debe comunicarse de inmediato.

Durante la operación, la pierna se mantiene en tracción, es decir, se tira de ella de forma constante para abrir la articulación. Esta presión puede irritar los nervios o afectar la piel cercana a la ingle o los glúteos. Si observa alguna zona de piel irritada o hormigueo en la ingle o zona genital, coméntele a su equipo médico.

Los instrumentos se introducen en la articulación a través de pequeñas incisiones; en ocasiones, el cartílago o el labrum pueden resultar dañados durante este proceso. Su cirujano actúa con sumo cuidado para evitarlo. Si tras unas semanas la cadera le duele más que antes, hable de ello en la revisión.

En raras ocasiones, la cadera puede volverse inestable tras la operación, e incluso dislocarse. Una dislocación significa que la cabeza femoral sale de la cavidad acetabular. Si de repente siente que la cadera “cede”, se bloquea o no puede moverla, acuda de inmediato a urgencias.

La fractura del fémur justo debajo de la cadera es una complicación muy poco frecuente. Un dolor intenso y repentino en el muslo o la ingle después de la cirugía, especialmente al soportar peso con la pierna, requiere atención urgente.

Tras cualquier intervención quirúrgica, pueden formarse coágulos sanguíneos en las venas de la pierna. Esté atento a cualquier hinchazón, calor o sensibilidad excesiva en la pantorrilla. Comunique estos síntomas sin demora; su equipo evaluará sus factores de riesgo y decidirá si necesita medicación para prevenir coágulos.

Las infecciones son poco comunes. Si la piel alrededor de las incisiones se enrojece, calienta, hincha o segrega líquido, o si presenta fiebre, contacte de inmediato a la clínica.

En algunos casos, la cadera se vuelve rígida o se forman pequeñas áreas de hueso nuevo en los tejidos blandos circundantes. Si nota un sonido de “clic” o fricción, o si el movimiento no mejora con el ejercicio, mencione esto en la revisión.

Una minoría de pacientes necesitan una cirugía adicional posteriormente, ya sea otra intervención artroscópica o, en ciertos casos, un reemplazo de cadera. Si el dolor regresa o no desaparece por completo, su cirujano lo reevaluará y le expondrá las opciones disponibles.

¿Cuándo debemos ser contactados?

La mayoría de los problemas aparecen en los primeros días o semanas. Llámenos si tiene fiebre, o si la piel alrededor de los pequeños cortes se vuelve roja, caliente, hinchada o comienza a supurar líquido. Llámenos si la pantorrilla se hincha, se calienta o se vuelve sensible de forma repentina, o si le cuesta respirar. Infórmenos sobre cualquier entumecimiento, hormigueo o debilidad nueva en la pierna que no mejore, o sobre cualquier zona dolorosa en la piel cerca de la ingle o los glúteos. Acuda a urgencias si experimenta un dolor intenso y súbito en la cadera, el muslo o la ingle, especialmente al apoyar peso en la pierna. Acuda a urgencias si la cadera cede de repente, se “traba” o no puede moverla.
