Qué está sintiendo

El dolor se localiza justo en la última articulación del dedo, la que está más cerca de la uña. La artritis por desgaste (osteoartritis) desgasta el cartílago liso que permite el deslizamiento de la articulación. Posteriormente, se forma hueso nuevo en los bordes de la articulación, lo que provoca el dolor, la rigidez y la hinchazón que usted percibe.

¿Qué está ocurriendo realmente?

Cada dedo está formado por una cadena de pequeños huesos. La articulación situada justo en la punta del dedo, junto a la uña, se denomina articulación distal del dedo. En esta afección, la artritis degenerativa afecta dicha articulación más que cualquier otra en la mano.

El cartílago sano actúa como un amortiguador entre los huesos; permite que la articulación se mueva suavemente al doblarse y enderezarse. Cuando el cartílago se desgasta, los huesos rozan entre sí. El cuerpo responde generando hueso adicional en los bordes de la articulación. Ese nuevo hueso, junto con la superficie articular irregular, provoca el dolor, la hinchazón y la rigidez que se sienten.

Los tendones también desempeñan un papel clave. Un tendón es un cordón resistente que conecta el músculo con el hueso. Estos cordones se extienden por la parte superior e inferior del dedo, y deben mantenerse en equilibrio para que la punta del dedo se doble y enderece normalmente. La artritis puede alterar ese equilibrio, de modo que la articulación queda en una posición doblada que, con el tiempo, empeora progresivamente.

Una lesión previa también puede predisponer a esta afección. Si alguna vez se sufrió un dedo en martillo, en el que el tendón encargado de enderezar la punta del dedo se desgarró o se separó, la articulación pierde su equilibrio. La punta del dedo queda caída, y la articulación media situada detrás puede enderezarse excesivamente, adoptando una forma similar al cuello de un cisne. Una articulación mantenida en esa posición durante años se desgasta prematuramente, provocando dolor y rigidez.

Existen dos patrones principales de esta artritis. En uno de ellos, la articulación presenta un aspecto y comportamiento típicos del desgaste degenerativo. En el otro, denominado artritis erosiva, la propia superficie articular se va desgastando gradualmente; este tipo suele ser más doloroso y destructivo.

La buena noticia es que esta articulación realiza funciones limitadas: ayuda en movimientos de pinza fina, pero no soporta la mayor parte de la fuerza de agarre. Por ello, las opciones de tratamiento van desde el control de los síntomas hasta la rigidez o sustitución de la articulación, según el grado de afectación por la artritis.

Qué podemos hacer al respecto

Qué esperar

Para la mayoría de las personas, esta artritis es una condición crónica y no algo temporal. El dolor y la rigidez suelen aparecer y desaparecer; a menudo empeoran después de usar mucho el dedo. El descanso suele aliviar los síntomas. Con el tiempo, la articulación puede volverse aún más rígida, y una flexión que no se endereza por completo puede ir aumentando gradualmente.

Si la artritis se originó a raíz de una lesión previa, como un dedo en martillo, el pronóstico es similar. La articulación puede perder algo de movilidad, pero esa pérdida no siempre afecta la sensación o el funcionamiento diario del dedo. Algunas personas conservan una buena funcionalidad incluso con una flexión limitada en la punta del dedo.

Sin tratamiento, los principales riesgos son el dolor persistente y una mayor rigidez o flexión de la articulación. Si una lesión grave en la articulación no se trata rápidamente, en un plazo de 8 horas, esta puede volverse muy rígida. Dejar una articulación doblada sin intervención durante años también puede provocar su desgaste prematuro.

Con tratamiento, la mayoría de las personas notan que el dolor disminuye. Medidas sencillas como el uso de una férula y la terapia de la mano suelen ser suficientes. Si se requiere cirugía, la artrodesis elimina el dolor pero deja la articulación rígida. Si para usted es más importante conservar el movimiento, el recorte de los bordes óseos desgastados o un reemplazo articular pueden aliviar el dolor manteniendo cierta movilidad. Cada una de estas opciones conlleva sus propios riesgos; su cirujano se los explicará detalladamente.

Un dato importante: si tanto la articulación media como la de la punta del mismo dedo presentan un dolor intenso, a veces es posible tratarlas en una sola intervención quirúrgica.

Cuándo consultar a un profesional

Acuda a su médico de cabecera si la articulación de la punta del dedo sigue doliendo a pesar del reposo, o si hay un bulto o quiste cerca de la uña que sigue creciendo o altera su forma. Solicite una evaluación especializada si la articulación se vuelve cada vez más rígida o se dobla más con el tiempo; si le resulta más difícil realizar tareas delicadas como abrochar botones o recoger monedas; o si una lesión previa de dedo en martillo ha dejado la articulación rígida o caída. Diríjase a urgencias si ha sufrido una compresión o lesión grave en la articulación de la punta del dedo, ya que un retraso superior a 8 horas puede provocar una rigidez importante.

La artritis de la articulación distal del dedo merece una lectura adicional, ya que la artrodesis, la intervención quirúrgica estándar y generalmente descrita como sencilla, presenta una tasa de complicaciones superior a la que su reputación sugiere; además, existe una alternativa que preserva el movimiento y rara vez se menciona.

La fusión no es tan sencilla como parece

La fusión de la articulación de la punta del dedo se presenta como una intervención fiable; en cuanto al alivio del dolor, efectivamente lo es. Sin embargo, el perfil de complicaciones no es tan benigno. Un estudio que analizó los factores de riesgo en 173 pacientes lo afirma claramente: la artrodesis de la articulación interfalángica distal suele generar complicaciones, siendo la artrosis, la artrodesis de revisión y el tabaquismo los factores de riesgo identificados [1].

El hecho de que el tabaquismo figure en esa lista merece atención, pues es el único factor que el paciente puede controlar. La consolidación ósea depende del riego sanguíneo; se trata de una articulación pequeña, rodeada de una capa delgada de tejidos blandos, situada al final del trayecto vascular del dedo.

El implante que logra una mejor unión también genera problemas que los implantes más baratos no causan

En el debate sobre los métodos de fijación, la respuesta es sorprendentemente clara: se trata de un intercambio, no de una solución definitiva. En un total de 1,125 pacientes, los tornillos de compresión sin cabeza parecen aumentar las tasas de unión ósea, pero se asocian a complicaciones que no se observan con otras técnicas más económicas y ya consolidadas; además, aparte de la unión ósea, no existen pruebas suficientes que demuestren la superioridad de dichos tornillos [2].

Las complicaciones específicas relacionadas con estos tornillos se deben a la anatomía. El tornillo discurre a lo largo del eje de la punta del dedo, por lo que pasa cerca del lecho ungueal y puede provocar deformidad de la uña; además, en falanges distales muy pequeñas puede no haber suficiente hueso para fijarlo adecuadamente. Los alambres son más baratos y evitan esos problemas específicos, aunque logran una unión ósea algo menos fiable.

Cuando el objetivo principal es lograr la unión ósea, realizar una fusión de revisión o tratarse de pacientes fumadores, la ventaja de los tornillos resulta más evidente. En cambio, cuando el hueso es pequeño y la integridad de la uña es importante, no queda tan claro que sean la opción adecuada.

La alternativa que preserva el movimiento

La fusión articular no es la única opción, aunque esta alternativa no se discute con frecuencia. En pacientes cuya principal queja son los bultos óseos y el dolor que estos provocan, más que la artritis en toda la articulación, se pueden extirpar los osteofitos prominentes manteniendo la integridad de la articulación.

La quilectomía abierta de la articulación de la punta del dedo se describe como una alternativa segura y eficaz a la artrodesis en pacientes con artrosis sintomática que desean conservar el movimiento articular, en un total de 78 pacientes [3].

Esto es importante porque la articulación de la punta del dedo contribuye poco a la fuerza de agarre, pero sí en gran medida a la manipulación fina y al aspecto estético de la mano. Para quienes sus síntomas se deben precisamente a los nódulos de Heberden, aceptar una articulación de la punta del dedo permanentemente rígida supone un sacrificio mayor de lo que parece; sin embargo, existe una opción intermedia.

El quiste que suele acompañarlo

La artritis en esta articulación suele generar un quiste mucoso, una pequeña protuberancia llena de líquido que se origina en la articulación afectada, generalmente al lado de la uña. Dado que depende de la articulación subyacente, en ese sentido se comporta como el ganglio de la muñeca: drenarlo reduce la hinchazón, pero no elimina la causa raíz. Este tema se aborda por separado, pero es importante conocer esta relación, ya que la aparición recurrente de este quiste es señal de la artritis subyacente y no un problema aislado.
