Una fractura del astrágalo es una rotura en un hueso pequeño situado en lo profundo del tobillo. El astrágalo se encuentra entre la tibia y el calcáneo, conectándolos a ambos. Debido a su posición entre estos dos huesos, el dolor suele sentirse en lo profundo del tobillo y del retropié, en lugar de localizarse en un punto concreto que se pueda señalar.

El dolor empeora al soportar peso con el pie. Caminar, estar de pie, impulsarse para subir escaleras o girar el tobillo suelen intensificar el dolor. El descanso con el pie elevado suele aliviarlo. Muchas personas notan el tobillo rígido al despertar o después de permanecer sentadas durante un rato.

Las tareas cotidianas que requieren un tobillo estable y móvil se vuelven difíciles. Es posible que le cueste caminar hasta el buzón, permanecer de pie junto a la encimera de la cocina o conducir. Empujar un carrito de la compra o levantarse del suelo también puede resultar complicado, ya que el pie no puede soportar el peso de manera uniforme.

Algunas fracturas del astrágalo vienen acompañadas de otras lesiones en el mismo tobillo. El daño en el cartílago del astrágalo es frecuente cuando el tobillo se fractura, y puede seguir causando dolor incluso después de que el hueso haya sanado. Si siente que el tobillo “cede” o sigue hinchándose tras una lesión, eso podría indicar una lesión cartilaginosa o ligamentos sueltos en la parte externa del tobillo. Estas fracturas pueden pasar desapercibidas con facilidad. Las radiografías simples omiten aproximadamente la mitad de las lesiones cartilaginosas del tobillo, incluso cuando se combinan con un examen físico. La tomografía computarizada muestra el hueso fracturado con gran detalle, pero no permite visualizar el cartílago. La resonancia magnética, en cambio, muestra tanto el cartílago como el hueso subyacente, y sus resultados coinciden estrechamente con lo que un cirujano observa al examinar la articulación mediante artroscopia.

¿Qué ocurre realmente?

El astrágalo es un hueso pequeño que se encuentra en un espacio reducido entre el hueso de la espinilla por arriba y el calcáneo por abajo. No cuenta con músculos propios y la mayor parte de su superficie está cubierta de cartílago liso. Imagíneselo como un trozo duro de tiza atrapado entre dos piezas: cuando el pie se eleva bruscamente y el tobillo queda comprimido, el hueso puede fracturarse entre los dos huesos que lo presionan.

Existen distintos tipos de fracturas según la fuerza aplicada. Un aterrizaje brusco con el pie flexionado hacia arriba y girado hacia adentro puede hacer que se desprenda un pequeño fragmento de hueso en el lado externo del astrágalo. Una torsión fuerte o un movimiento excesivo del pie en una dirección determinada puede provocar la separación de fragmentos pequeños en la parte posterior del astrágalo. Cuando el tobillo sufre una compresión directa desde arriba, el cuerpo principal del astrágalo puede partirse; si al mismo tiempo se fractura el maléolo medial, la grieta puede extenderse a ambos huesos.

El problema de estas fracturas radica en lo que alteran. El astrágalo contribuye a formar dos articulaciones funcionales; cualquier fragmento que se desprenda puede quedar suelto dentro de la articulación, a veces incluso volteado en el hueco del que provino. Estos fragmentos sueltos y las superficies articulares irregulares generan fricción en lugar de un deslizamiento fluido, motivo por el cual el tobillo permanece rígido y doloroso mucho tiempo después de la lesión. Si la fractura está muy desplazada o el hueso se sale por completo de la articulación, el riego sanguíneo al astrágalo puede verse afectado, provocando la necrosis de parte del hueso. El daño al cartílago y la falta de alineación articular favorecen la aparición de artritis degenerativa en el tobillo con el paso del tiempo.

Por este motivo, estas fracturas se consideran graves incluso cuando parecen menores. Una pequeña grieta en el lado externo del astrágalo es una lesión frecuente en el snowboard; si no se detecta o se trata tardíamente, la articulación situada debajo del tobillo puede deteriorarse gravemente. Volver a colocar los fragmentos en su posición original y alinear adecuadamente las superficies articulares brinda al tobillo la mejor oportunidad de recuperar su movilidad normal y sin dolor.

Qué podemos hacer al respecto

La duración del tratamiento con yeso depende del tipo de fractura. En ciertas fracturas estables del tobillo, tres semanas con yeso o aparato ortopédico producen resultados similares a seis semanas en cuanto a los síntomas y al estado del tobillo al cabo de un año; por eso no siempre es necesario mantener el tratamiento por más tiempo. Una vez que el hueso se ha consolidado, se inicia la fisioterapia para recuperar el movimiento, la fuerza y el equilibrio.

Si la fractura ha desplazado los fragmentos óseos o si la superficie articular ya no es lisa, normalmente se recomienda la cirugía de inmediato. El objetivo es volver a unir los fragmentos óseos y restaurar la forma del astrágalo para que las superficies articulares vuelvan a alinearse. Dado que cada patrón de fractura es distinto, planificamos la intervención con sumo cuidado antes de proceder; ese plan determina la vía de acceso al hueso. Algunas fracturas requieren el uso de placas o pequeños alambres para mantener los fragmentos en su sitio mientras sanan. Si un fragmento de cartílago y hueso se ha desprendido dentro de la articulación, podemos volver a colocarlo o alisar la superficie. Cuando una lesión cartilaginosa no responde a tratamientos menos invasivos, se puede trasplantar un pequeño fragmento de su propio hueso y cartílago en la zona dañada.

En algunos casos, pese a un tratamiento inicial adecuado, pueden surgir complicaciones posteriormente. Si parte del astrágalo pierde el aporte sanguíneo y muere, o si el tobillo adopta una forma dolorosa y degenerada, aún existen opciones terapéuticas. Entre ellas se incluyen la limpieza articular, el injerto óseo o la fusión de huesos para evitar el roce entre superficies dolorosas. Qué esperar

La mayoría de las fracturas de astrágalo sanan con tratamiento, y muchas personas vuelven a caminar y a realizar sus actividades cotidianas. Algunas fracturas son más graves que otras. Si el hueso quedó muy desplazado o salió por completo de la articulación, parte del astrágalo puede perder su irrigación sanguínea y morir; con el tiempo, el tobillo puede desarrollar artritis por desgaste. Incluso con un buen tratamiento, estos problemas siguen siendo frecuentes; un pequeño grupo de pacientes con lesiones graves sufre dolor y rigidez crónicos que limitan sus actividades. El sobrepeso también influye: las personas con obesidad suelen presentar más dolor, peor funcionalidad y mayores dificultades en su vida diaria tras el tratamiento de una fractura de tobillo.

Si los fragmentos óseos nunca se volvieron a colocar o si el tratamiento se retrasó, el pronóstico suele ser peor. Una articulación que no está bien alineada no desliza sino que se frota, y el daño cartilaginoso resultante puede seguir provocando dolor mucho después de que el hueso haya sanado. Las pequeñas fracturas en los bordes externo o posterior del astrágalo pueden pasar inadvertidas; si se detectan tarde o no se tratan adecuadamente, pueden causar discapacidad duradera en la articulación situada por debajo del tobillo.

La recuperación es gradual, no rápida. Durante un tiempo se debe evitar cargar peso sobre el pie mientras el hueso sana; luego se trabaja en el movimiento, la fuerza y el equilibrio. El tobillo puede permanecer rígido e hinchado durante meses, y puede tomar tiempo volver a confiar en él en terrenos irregulares. Algunas personas recuperan el mismo nivel deportivo que tenían antes de la lesión; otras quedan con un tobillo que les duele tras jornadas largas o con el frío. Su cirujano le explicará cuál de estos desenlaces es más probable según su tipo concreto de fractura, y qué se puede hacer si el tobillo no evoluciona como se esperaba.

Cuándo consultar a un especialista

Acuda de inmediato a un médico tras cualquier caída fuerte, torsión, caída desde cierta altura o accidente de tráfico que provoque un dolor intenso en el tobillo; especialmente si el retropié se hincha rápidamente o el tobillo parece inestable. No espere a ver si el dolor desaparece por sí solo. Solicite una evaluación especializada si su tobillo sigue cediendo, sigue hinchado o el dolor no mejora con el reposo, ya que el daño en el cartílago y la laxitud de los ligamentos laterales del tobillo son comunes en estas lesiones y fácilmente pasan desapercibidos.
