¿Por qué se ha recomendado esta operación?¶
El Dr. Kieran Hirpara, cirujano de extremidades superiores en el Mater Private Hospital Rockhampton, comienza por proponer las opciones menos invasivas que se adapten a su condición. Por lo general, los pacientes son derivados a nuestra clínica por su médico de cabecera; si un fisioterapeuta le ha sugerido que nos consulte, igualmente necesitará una derivación de su médico de cabecera para poder acceder al reembolso de Medicare. En su consulta, tomamos su historia clínica, examinamos su mano y, si es necesario, solicitamos estudios de imagen para confirmar el diagnóstico.
La artritis en la base del pulgar es un problema crónico derivado del desgaste, por lo que normalmente intentamos primero tratamientos no quirúrgicos. Estos pueden incluir cambios en las actividades cotidianas, terapia de mano o el uso de una férula. La cirugía se considera cuando estas medidas no logran mejorar suficientemente sus síntomas.
La trapeziectomía consiste en la extirpación de un pequeño hueso situado en la base del pulgar que se ha deteriorado. Se trata de una operación sencilla y de larga trayectoria para esta patología, con buenos resultados a largo plazo y un perfil de efectos secundarios aceptable. La indicamos cuando la artritis en la base del pulgar provoca dolor persistente o dificultades para agarrar objetos y realizar movimientos finos de la mano, que no mejoran con otros tratamientos. El objetivo principal de la operación es aliviar el dolor. Otros objetivos que perseguimos son mantener el pulgar móvil, fuerte y estable.
Antes de la operación¶
Antes de la cirugía, su cirujano le dará instrucciones claras que deberá seguir. Deberá dejar de comer y beber siete horas antes de la operación; esto permite cierta flexibilidad en el programa quirúrgico si algo avanza antes de lo previsto. Lleve consigo una lista de todos los medicamentos que toma actualmente, ya que el cirujano le indicará cuáles debe suspender. Organice que alguien lo lleve a casa después de la intervención, pues no podrá conducir por sí mismo. El día de la operación, use ropa holgada y cómoda. Por lo general, se requieren radiografías para planificar la cirugía; en ocasiones también una resonancia magnética o una ecografía. Si padece otras enfermedades, es posible que necesite análisis de sangre o una consulta con el anestesista.
El día de la intervención¶
Llegará a la unidad de admisiones quirúrgicas del hospital, donde se le registrará y preparará para la cirugía. Allí conocerá al anestesista. Esta operación se realiza bajo anestesia general; usted estará completamente dormido durante todo el procedimiento. En algunos pacientes también se aplica un bloqueo nervioso regional para aliviar el dolor postoperatorio; el anestesista decide al respecto ese mismo día según sus circunstancias individuales. Posteriormente, se le lleva al quirófano, donde se realiza la operación.
Despertará en la sala de recuperación, donde las enfermeras le vigilarán mientras la anestesia va desapareciendo. Una vez que su estado sea estable, será trasladado a la planta de hospitalización o podrá volver a casa, según el tipo de intervención y su recuperación.
Qué implica la operación¶
La trapeziectomía consiste en la extirpación de un pequeño hueso situado en la base del pulgar, el cual se ha deteriorado debido a la artritis. El cirujano realiza una única incisión en la zona a operar para acceder al hueso. Una vez extraído, el espacio resultante se llena con el tiempo de tejido cicatricial, proporcionando así un nuevo soporte al pulgar.
En algunas intervenciones se añaden pasos adicionales. Una opción es reconstruir una estructura de soporte utilizando un tendón cercano, colocando luego un fragmento enrollado de dicho tendón en el espacio donde antes estaba el hueso. Esto se conoce como reconstrucción ligamentosa con interposición tendinosa. Antes de que firme el formulario de consentimiento, su cirujano le explicará si este tipo de procedimiento forma parte de su plan de tratamiento.
La herida se cierra con puntos de sutura. Posteriormente, se coloca la mano en una férula rígida de yeso durante 1 semana. Se le enseñará cómo mantener la mano elevada y cómo mover los dedos, la articulación distal del pulgar, el codo y el hombro durante el proceso de recuperación.
Después de la operación¶
La mayoría de los pacientes permanecen una noche en el hospital tras esta operación, aunque algunos pueden volver a casa el mismo día. Despertará en la sala de recuperación y luego será trasladado a la habitación. Las enfermeras lo vigilarán y le administrarán medicamentos para el dolor. Su mano quedará inmovilizada con una férula posterior; deberá mantenerla elevada y mover los dedos, el pulgar, el codo y el hombro tal como se le indicó. Alguien debe acompañarlo durante las primeras 24 horas después de llegar a casa. Dejamos el vendaje puesto durante unos 10 días; por favor, no lo retire antes de ese plazo a menos que se lo indiquemos. Lo cambiamos o lo retiramos cuando venga a la consulta.
Recuperación¶
Durante los primeros días, su pulgar y la base de la mano estarán adoloridos e hinchados. Esto forma parte normal del proceso de curación. Mantener la mano elevada sobre almohadas, incluso mientras descansa o duerme, ayuda a reducir la hinchazón. Los analgésicos que le hayan recetado aliviarán el malestar; la mayoría de las personas notan una mejoría constante durante las primeras dos semanas.
Al principio, la mano permanecerá inmovilizada con una férula posterior, pero deberá mover los dedos, el pulgar, el codo y el hombro tal como le enseñaron en el hospital. Una vez que le retiren el vendaje en la consulta de seguimiento, comenzará la terapia de mano con Ruby Doolan en Extend Rehabilitation. Ruby es terapeuta especializada en mano: ella le guiará en los ejercicios y confeccionará cualquier férula que necesite a medida que su pulgar recupere movilidad y fuerza. Diariamente realizará movimientos suaves en casa, progresando gradualmente hacia la capacidad de agarrar y pellizcar según lo permita su pulgar. Actividades cotidianas como escribir, sostener una taza o girar una llave volverán poco a poco, una por una.

La recuperación varía según cada persona. Su cronograma podría ser distinto; su cirujano y terapeuta de mano le guiarán en cada etapa del proceso.
Qué puede salir mal¶
La mayoría de los pacientes evolucionan bien, pero en ocasiones pueden surgir problemas. Su cirujano y el equipo lo vigilarán de cerca para detectar cualquier incidencia a tiempo.
A veces, un nervio cercano al pulgar se irrita después de la cirugía. Es posible que note hormigueo, ardor o entumecimiento en el dorso del pulgar y la mano. Si esto ocurre, normalmente desaparece por completo en el transcurso de un año. Coméntele al médico en su revisión para que pueda hacer un seguimiento.
El pin metálico que a veces se utiliza para fijar los huesos durante la cicatrización puede generar problemas. La piel alrededor de él puede volverse sensible o irritarse. También puede aflojarse o desplazarse de su posición, y la zona circundante puede infectarse, aunque esto es poco frecuente. Si el pin se siente suelto, se mueve, o la piel alrededor presenta enrojecimiento o secreción, llame a la clínica. Un pin que se ha aflojado o desplazado suele requerir su extracción precoz; hacerlo no afecta negativamente el resultado quirúrgico.
La infección es un riesgo en cualquier cirugía. Esté atento a dolores que empeoren en lugar de mejorar, enrojecimiento que se extienda desde la herida, hinchazón progresiva o secreción líquida desde el corte. Si observa alguno de estos signos, comuníquese con la clínica de inmediato.
Si su operación incluyó la colocación de una prótesis articular o un espaciador en lugar de una simple resección ósea, hay otros aspectos que debe vigilar. La prótesis puede aflojarse, hundirse en el hueso adyacente, inclinarse o desplazarse de su posición. Es posible que sienta nuevo dolor, crujidos o una sensación de inestabilidad en el pulgar. En caso de infección de la articulación implantada, a veces es posible tratarla extrayendo la prótesis y realizando la intervención más sencilla de resección ósea. Comuníquele cualquier síntoma nuevo o que vaya cambiando en su próxima revisión.
Si se cae sobre la mano operada, o si el pulgar presenta de repente dolor, hinchazón o deformidad tras un golpe, busque atención médica sin esperar a su próxima cita.

Bony Anatomy & Architecture¶
- The thumb metacarpal is independent and articulates with the trapezium [35].
- The hand consists of 19 bones, 17 articulations, and 19 muscles situated entirely within the hand [30].
- The thumb ray is the shortest and is clearly separated from the fingers, implanted proximally [30].
- The thumb has a more proximal and lateral position, allowing movement inward and outward from the palm [30].
- The web space of the thumb is the largest and deepest among the hand's web spaces [30].
- The thumb metacarpal base is the attachment site for suture button suspensionplasty devices [42].
- The second metacarpal serves as the distal attachment site for suture button suspensionplasty devices [42].
- Drilling the second metacarpal for suture button placement carries a risk of metacarpal fracture if the drill is placed too far dorsally [42].
Vascular Anatomy¶
- The "princeps pollicis" artery is the terminal branch of the radial artery [36].

Investigations¶
- The trapezium is clearly angled out in front of the carpal plane so that the first metacarpal makes an angle of about 45 degrees with the second metacarpal in the sagittal plane [40].
- The articular surfaces of the trapezium and the base of the first metacarpal have a saddle shape in opposing planes [40].
- The thumb metacarpal is the shortest metacarpal [40].
- The first ray is made up of only three bones: a metacarpal and two phalanges [40].
- The first ray continues the external column of the carpus formed by the scaphoid and trapezium [40].
- The scaphotrapezial joint is susceptible to degenerative change after partial trapeziectomy, with an average participant demonstrating stage 1 disease (minimal narrowing on the radiograph) at an average 9-year follow-up [17].
- There was no symptomatic progression of arthritis at the scaphotrapezial joint after partial trapeziectomy in patients with no preoperative symptoms of scaphotrapezial joint arthritis [17].
- The radiographic presence of scaphotrapezoidal arthritis does not correlate with the patient's main symptoms after partial trapeziectomy [69].
Treatment¶
Operative Technique and Variations¶
- Trapeziectomy is performed via a dorsoradial incision, involving release of the first extensor compartment and passing a distally based strip of the abductor pollicis longus (APL) through a slit in the flexor carpi radialis to form a suspensory support [27].
- The trapezium is removed through a dorsal approach, with the first dorsal compartment released along the dorsal margin of the subsheath to prevent volar tendon subluxation [63].
- The trapezium may be removed piecemeal with a Rongeur or as a whole with a corkscrew joystick, depending on surgeon preference [63].
- A dorsal incision centered over the thumb carpometacarpal joint is used for suture button suspensionplasty, with the surgical plane between the abductor pollicis longus and extensor pollicis brevis tendons [70].
- In suture button suspensionplasty, the trapezium is removed piecemeal to protect the underlying flexor carpi radialis tendon [70].
- Osteophytes around the first and second metacarpal bases are removed during suture button suspensionplasty to prevent impingement when Mini TightRope devices are placed and tied [70].
- A corkscrew found in the Mini-TightRope CMC set is used to gain solid purchase in the trapezium as a joystick for excision of the trapezium as a whole [71].
- A C-ring targeting guide is used routinely in suture button suspensionplasty to ensure the exit point on the second metacarpal is placed at the ulnar surface of the diaphyseal-metaphyseal junction [71].
- A tapered suture-passing guidewire that is 1.1 mm at its widest diameter and tapers to 0.86 mm with a Nitinol loop is used in suture button suspensionplasty [71].
- Wide-awake basal joint trapeziectomy can be performed using only locally injected lidocaine and epinephrine without a tourniquet or sedation [29].
- Outcomes for the anterior approach to trapeziectomy are equally good or better than with the posterior approach [41].
- Arthroscopic hemitrapsiectomy and thermal capsular modification without interposition is a treatment option for stage III thumb carpometacarpal arthritis [13].

Outcomes and Efficacy¶
- Trapeziectomy is a good method of treating osteoarthritis of the thumb base [41].
- The outcomes of trapeziectomy, trapeziectomy with tendon interposition, and trapeziectomy with ligament reconstruction and tendon interposition were similar after a minimum follow-up of 5 years [6].
- Arthroscopic hemitrapsiectomy and thermal capsular modification without interposition results appear to last for a minimum of 4 years and are comparable to those reported for open techniques involving complete trapeziectomy [13].
- Trapeziectomy and LRTI are effective procedures for patients aged less than 56 years [23].

Biomechanics and Adjuncts¶
- Biomechanic analysis found several advantages to the tie-in trapezium implant compared with LRTI, including reduction in axial and radial displacement and maintenance of the trapezial space [24].
- The value of ligament reconstruction and temporary stabilisation of the pseudarthrosis with a Kirschner wire remains unproven until further larger studies are performed [1].
- Trapeziectomy combined with Swanson implant gives better results in the short term if there are no complications of the operation [10].
- These results suggest an advantage of simple trapeziectomy over trapeziectomy with ligament reconstruction and tendon interposition, however, further study is warranted [11].
Complications¶
General Complications¶
- Potential complications of trapeziometacarpal arthroplasty techniques include infection, sensory nerve irritation, radial artery injury, injury to the flexor carpi radialis tendon during bone removal, and postoperative subsidence with weakness and recurrence of pain [60].
- Persistent post-operative pain following trapeziectomy can be caused by incomplete trapezial resection, neuroma formation due to injury of sensory branches of the radial nerve, unrecognized concomitant arthritis in the scaphotrapezoid joint, metacarpophalangeal arthritis, metacarpal base spurs, instability in the metacarpophalangeal joint, and complex regional pain syndrome [66].
- Persistent post-operative pain following trapeziectomy can be caused by abutment between the metacarpal base and scaphoid due to proximal migration [66].
Nerve and Soft Tissue Complications¶
- Five patients developed transient superficial radial nerve neuritis that resolved over 3 months following arthroscopic partial trapeziectomy with soft tissue interposition [64].
- Patients should be warned about the potential complication of flexor carpi radialis tendinitis after trapeziectomy and abductor pollicis longus suspensionplasty [77].
Bony Complications¶
- Index metacarpal fracture after suture button suspensionplasty has been reported [60].
- Surgeons should use caution when considering suture button fixation after complete trapeziectomy until more data are available regarding index metacarpal fracture [19].
Functional and Deformity Complications¶
- One patient developed a 30-degree hyperextension deformity at the metacarpophalangeal joint requiring metacarpophalangeal joint arthrodesis following arthroscopic partial trapeziectomy with soft tissue interposition [64].
- One patient developed painful instability of the thumb metacarpal base with radial and proximal migration of the thumb metacarpal following arthroscopic partial trapeziectomy with soft tissue interposition [64].
Comparative Complication Rates¶
- Increased complications were observed in trapeziectomy with ligament reconstruction and tendon interposition compared with trapeziectomy alone [11].
- Patients who undergo arthrodesis have higher reoperation rates and incidence of postoperative complications than those who undergo ligament reconstruction and tendon interposition [80].
- Complications were more frequent following arthrodesis than ligament reconstruction and tendon interposition, although most did not affect the overall outcome [79].
- Results for a large series of suture button suspension for carpometacarpal arthroplasty with intermediate follow-up revealed low complication rates [78].
Recovery¶
- A postoperative regimen with early mobilization after trapeziectomy is as safe and effective as a postoperative regimen with longer immobilization in patients with first carpometacarpal osteoarthritis [54].
