Qué está sintiendo

Se trata de un dolor en el lado del meñique, en la parte posterior de la muñeca: el lado más cercano a donde se coloca la correa del reloj cuando lo lleva en esa posición. Por lo general, el dolor es más intenso al girar la palma de la mano hacia arriba y doblar la muñeca hacia el meñique; esto es exactamente lo que ocurre al realizar un golpe de revés en tenis, un swing de golf o al retorcer un paño para escurrirlo.

Existen dos variantes de este problema; muchas personas presentan síntomas de ambas.

Tendón irritado. El dolor se manifiesta como una molestia sorda a lo largo del tendón; a veces hay hinchazón palpable. También duele presionar la muñeca contra una resistencia. Este tipo de dolor suele ir aumentando con el paso de las semanas debido al uso repetido, en lugar de aparecer de forma repentina.

Tendón que se desplaza de su ranura. En este caso, al girar el antebrazo se percibe un chasquido, clic o ruido característico; a menudo el propio paciente puede reproducirlo. Generalmente, este síntoma aparece tras algún incidente específico, como cuando el palo de golf golpea el suelo o tras un fuerte golpe de revés. Puede resultar alarmante, pues da la impresión de que algo se está “desprendiendo”.

¿Qué está ocurriendo realmente?

El tendón afectado es el extensor carpi ulnaris, comúnmente abreviado como ECU. Este tendón se extiende desde el músculo del antebrazo hasta el lado del meñique de la muñeca, situándose en un surco poco profundo en el extremo del cúbito, el hueso del antebrazo en ese lado. Un pequeño revestimiento de tejido, una especie de “túnel” propio, mantiene al tendón en dicho surco.

Ese surco es realmente poco profundo; su profundidad promedio es de apenas un milímetro y medio, por lo que el revestimiento es el encargado de mantener al tendón en su lugar. Si dicho revestimiento se desgarra o se estira —generalmente a causa de una torsión brusca con la palma hacia arriba y la muñeca flexionada—, el tendón puede salirse del surco y volver a entrar en él. Por otro lado, si el tendón y su revestimiento se irritan y engrosan debido a cargas repetidas, su deslizamiento se vuelve doloroso; esto corresponde a la versión tendinopática de la lesión.

Un dato importante que hay que conocer, pues influye en la interpretación de las pruebas: cierto grado de movimiento del tendón es totalmente normal. Cuando los investigadores examinaron a 755 personas sin ningún síntoma en la muñeca, aproximadamente 1 de cada 15 presentaba un movimiento brusco del ECU; de esas personas, casi la mitad mostraban en ecografías cómo el tendón salía visiblemente de su surco, sin dolor ni complicaciones. Por lo tanto, la presencia de dicho movimiento por sí sola no indica que haya algún problema.

¿Qué podemos hacer al respecto?

En el Mater Private Hospital Rockhampton, el Dr. Kieran Hirpara basará el diagnóstico principalmente en la exploración clínica y no en estudios por imagen; merece la pena explicar por qué.

Los estudios por imagen requieren una interpretación cuidadosa en este caso. En un estudio con más de 4.000 resonancias magnéticas de muñeca, se observaron alteraciones en el tendón ECU en aproximadamente 1 de cada 8 exámenes, incluso en personas a quienes se realizó el estudio por otras razones. Si se detecta una anomalía en el ECU mediante resonancia magnética, la probabilidad de que esa persona realmente presente problemas en dicho tendón es de apenas el 6%. La resonancia magnética sí resulta útil para descartar otras causas de dolor en esa zona de la muñeca —por ejemplo, un desgarro de cartílago—, pero la presencia de una alteración en el tendón por sí sola no constituye un diagnóstico definitivo. La ecografía suele ser más provechosa, ya que permite observar el movimiento del tendón en tiempo real mientras el paciente reproduce el movimiento que provoca el dolor.

La mayoría de las personas mejoran sin necesidad de cirugía. El tratamiento suele comenzar con la modificación de las actividades que desencadenan los síntomas, acompañada de el uso de una férula o yeso durante un período determinado. En casos donde el tendón se desplaza fuera de su ranura, la posición es crucial: normalmente se mantiene la muñeca con el antebrazo girado palma abajo y la muñeca ligeramente flexionada hacia atrás, durante unas cuatro a seis semanas; esta posición es lo opuesto a aquella que facilita el desplazamiento del tendón.

La inyección de corticoides en la vaina tendinosa constituye un paso razonable en casos de tendinitis irritativa. En el mejor estudio disponible, los 13 pacientes con diagnóstico confirmado experimentaron algún alivio; en 10 de ellos el alivio fue total, y ninguno necesitó someterse a cirugía; la mayoría seguía sin molestias cuatro o cinco años después. Un indicador útil: si la primera inyección resuelve completamente los síntomas, es probable que el efecto perdure. Si, en cambio, solo produce un alivio parcial, con frecuencia persiste cierto dolor residual.

Limitamos el número de inyecciones a un máximo de tres aproximadamente; además, somos más cautelosos en atletas que practican lanzamientos o deportes con raqueta, pues existe la preocupación —aunque no respaldada por datos estadísticos, sí documentada en la literatura— de que la administración repetida de corticoides cerca de este tendón podría favorecer su eventual rotura.

La cirugía se reserva para aquellos pacientes cuya inestabilidad tendinosa sigue generando dolor a pesar de haber seguido un tratamiento adecuado con yeso, generalmente tras un período de dos meses. La intervención consiste en reconstruir o reforzar la vaina que mantiene al tendón en su ranura. Existen varias técnicas quirúrgicas, y todas arrojan buenos resultados.

Qué esperar

La recuperación mediante tratamiento no quirúrgico depende del grado de irritación del tendón. Los atletas con un problema leve suelen volver a la actividad en dos o tres semanas; los casos más graves requieren de seis a ocho semanas; y un desplazamiento completo tratado con yeso puede tardar de cinco a seis meses en permitir el regreso a deportes exigentes como el tenis.

Tras la cirugía, se espera un período de cuatro a seis semanas con el antebrazo inmovilizado en posición de palma hacia abajo mediante yeso o férula; posteriormente se recupera gradualmente el movimiento, comenzando el fortalecimiento a partir de los dos meses, y el entrenamiento específico para el deporte se inicia a los tres o cuatro meses. Los estudios publicados indican que los pacientes vuelven a sus actividades habituales alrededor de los dos meses y medio a tres meses y medio.

Dos aspectos importantes sobre la cirugía: primero, prácticamente todos los estudios publicados afirman que el fenómeno de “clic” no vuelve a aparecer; sin embargo, el único estudio que realizó escaneos posteriores constató que el tendón permanecía fuera de su ranura en aproximadamente la mitad de los pacientes, aunque estos estaban satisfechos y obtenían buenos resultados. En otras palabras, la operación alivia de forma fiable los síntomas, pero no siempre devuelve la anatomía a su estado original. Segundo, las mejoras en la fuerza de agarre y en el rango de movimiento son modestas; la cirugía se realiza para eliminar el dolor y el fenómeno de “clic”, no para fortalecer la muñeca.

Las complicaciones son poco frecuentes pero posibles: entumecimiento o sensibilidad en la zona debido a la irritación de un pequeño nervio cutáneo en el dorso de la muñeca, rigidez y, en casos excepcionales, un síndrome de dolor persistente.

Si el fenómeno de “clic” no le causa dolor, probablemente no sea necesario corregirlo. Las personas con este problema pero sin dolor suelen seguir practicando su deporte a alto nivel mediante el uso de cintas adhesivas y un seguimiento médico regular.

¿Cuándo consultar a un especialista?

Acuda a su médico de cabecera si el dolor en el lado del meñique de la muñeca no mejora tras unas cuantas semanas de reducir la actividad que lo provoca, o si le impide practicar deporte o trabajar.

Solicite una opinión de un cirujano de mano si al girar el antebrazo la muñeca hace un chasquido o crujido doloroso, si el dolor apareció tras una lesión por torsión específica, o si ya ha probado el reposo, una férula y una inyección sin obtener resultados duraderos.

Busque atención médica de inmediato si la muñeca se vuelve caliente, roja e hinchada; si aparece entumecimiento o debilidad en la mano; o si notó una pérdida súbita de fuerza con sensación de “ceder” en el momento — este último síntoma puede indicar que el tendón se ha roto.

En mayor profundidad

Esta sección profundiza más de lo necesario para que usted tome sus propias decisiones de tratamiento. Los problemas del músculo extensor carpi ulnaris merecen una lectura adicional, ya que ambos hallazgos que suelen confirmar el diagnóstico —una señal anormal en la resonancia magnética y un tendón que “chasquea” al girar la muñeca— son frecuentes incluso en personas sin ningún síntoma.

Un resultado anormal en la resonancia magnética no significa tanto como parece

El tendón ECU discurre por un surco en la cara posterior de la ulna, en el lado del dedo meñique de la muñeca. Cuando una persona siente dolor en esa zona y el estudio de resonancia muestra cambios en la señal de dicho tendón, la conclusión parece evidente.

Sin embargo, los datos contradicen esta interpretación. Tras analizar 4,301 resonancias magnéticas de muñeca, se observó que los cambios en la señal del tendón ECU son frecuentes y, con frecuencia, asintomáticos; además, dichos cambios raramente se asocian a tendinopatía sintomática, lo que genera un valor predictivo positivo muy bajo [1].

El “valor predictivo positivo bajo” es la clave aquí: significa que, de todas las muñecas que presentan este hallazgo, solo una minoría lo tienen como causa de su dolor. La resonancia magnética por sí sola no puede establecer el diagnóstico; debe coincidir con la presencia de sensibilidad exactamente en esa zona y con el dolor que se reproduce al realizar movimientos resistidos en la dirección adecuada.

El fenómeno de “clic” es común en personas sin síntomas

El segundo signo confirmatorio es el hecho de que el tendón “haga clic” de forma visible o audible al salirse de su ranura durante la rotación del antebrazo. Al examinar a 755 personas asintomáticas, se observó que la aparición de este fenómeno es tan frecuente que no puede considerarse un marcador fiable de patología por sí solo [2].

En la práctica, si el paciente percibe este “clic” en una muñeca que también le duele exactamente en ese punto, el hallazgo sí es significativo. En cambio, si el “clic” se detecta de forma incidental al examinar una muñeca que duele en otras zonas, probablemente se trate de una variante normal.

Considerando también los resultados de la resonancia magnética, el patrón observado es coherente: este tendón genera resultados anómalos en muñecas aparentemente sanas. Por ello, el diagnóstico depende más de la correlación entre todos los hallazgos que de cualquiera de ellos por separado.

Esto es importante porque la muñeca cubital cuenta con numerosas posibles fuentes de dolor.

El tendón ECU se encuentra justo al lado del TFCC, de la articulación radiocubital distal y de la articulación pisotriquetral; todas ellas generan dolor en la misma zona reducida. Es fácil atribuir ese dolor a un tendón ECU de aspecto anormal; sin embargo, según las cifras mencionadas, esto suele ser incorrecto. Este es el motivo principal por el cual se debe actuar con cautela antes de considerarlo como la causa del problema.

El tratamiento conservador es la base del tratamiento y resulta eficaz

Cuando el diagnóstico es correcto, el tratamiento no resulta espectacular. Una combinación de medidas conservadoras y rehabilitación puede lograr que los pacientes queden libres de síntomas y les permita volver a realizar las actividades que desean [3].

Dos características propias de este tendón determinan la rehabilitación. Dado que soporta cargas derivadas de la rotación del antebrazo combinada con desviación cubital —posición que se da en el swing de golf, el golpe con raqueta o al levantar una olla pesada—, modificar las actividades implica cambiar la dirección de la carga en lugar de simplemente guardar reposo. Además, como el tendón se encuentra dentro de una subvaina que puede estirarse o desgarrarse, permitiendo su subluxación, la inmovilización suele realizarse en pronación con la muñeca extendida; esto mantiene al tendón en su ranura mientras la vaina cicatriza.

Section 6 -- Term and Termination.
