¿Por qué se ha recomendado esta operación?

El Dr. Kieran Hirpara, cirujano especializado en cadera y rodilla en el Mater Private Hospital Rockhampton, recomienda la artroplastia total de cadera cuando el dolor en la cadera le impide llevar una vida normal. Antes de la operación

El día de la intervención

Qué implica la operación

El reemplazo total de cadera consiste en retirar las superficies articulares desgastadas y sustituirlas por piezas artificiales. El cirujano extrae la cabeza femoral y la cavidad acetabular dañadas y coloca en su lugar nuevas superficies metálicas y de plástico. Estas nuevas piezas se fijan firmemente al hueso para que puedan soportar el peso del paciente.

Existen varias formas de acceder a la articulación de la cadera; la incisión puede realizarse en la parte delantera o posterior de la cadera. Su cirujano elegirá el método que mejor se adapte a su cuerpo y a su experiencia. No existe un único método adecuado para todos los pacientes; la elección depende de sus necesidades, la calidad de sus huesos y la formación del cirujano.

También se eligen las piezas específicas para usted. Algunas se fijan al hueso mediante un cemento especial, mientras que otras cuentan con una superficie recubierta sobre la cual su propio hueso crece con el tiempo. El cirujano selecciona el tipo de fijación según su edad, nivel de actividad y calidad ósea. Una vez que la nueva articulación queda colocada y funciona correctamente, el cirujano cierra la incisión con puntos de sutura y la cubre con un vendaje. Deberá mantener ese vendaje durante unos 10 días, tal como se explica en la sección “Después de la operación”.

En total, la operación suele durar entre una y dos horas.

Después de la operación

Despertará en la sala de recuperación y luego será trasladado a la habitación. Las enfermeras controlarán su dolor periódicamente y le administrarán medicación para mantenerlo cómodo. La zona de la incisión en su cadera estará cubierta con un vendaje; lo dejaremos puesto durante unos 10 días. Por favor, no lo retire antes de ese plazo a menos que se lo indiquemos. Lo cambiaremos o lo retiraremos cuando vengamos a verle. No necesitará ninguna férula ni almohada especial para el cadera. La mayoría de los pacientes se ponen de pie y dan algunos pasos con ayuda de un andador el mismo día de la cirugía; el equipo médico le ayudará al iniciar la movilización. Recuperación

La recuperación varía de una persona a otra; su cronograma personal puede diferir del de los demás. Su cirujano y fisioterapeuta lo guiarán durante todo el proceso.

En los primeros días y semanas, es normal experimentar algo de dolor e hinchazón alrededor de la cadera. Esto forma parte del proceso de curación y suele disminuir gradualmente a medida que la articulación se recupera. Los analgésicos le ayudarán a sentirse cómodo, y su equipo médico los ajustará según sea necesario. Los movimientos suaves y los ejercicios que le indique su fisioterapeuta también reducirán la rigidez y la hinchazón.

Poco después de la cirugía, comenzará a caminar con ayuda de un dispositivo de soporte, progresando gradualmente a partir de ahí. Su fisioterapeuta le asignará ejercicios para realizar en casa; estos ayudan a recuperar la movilidad y la fuerza de la cadera. No será necesario usar un soporte ni una almohada especial. Podrá moverse por su hogar según se sienta capaz, siempre respetando las precauciones indicadas por su equipo médico. Con el paso de las semanas, tareas sencillas como vestirse o desplazarse por la casa le resultarán más fáciles.

Algunas personas regresan a casa el mismo día de la cirugía, mientras que otras permanecen una o dos noches en el hospital. En cualquier caso, comenzará a moverse y a realizar los ejercicios desde el principio. Una vez que su cirujano considere que la cadera está sanando adecuadamente, podrá volver a conducir y retomar otras actividades poco a poco. Muchas personas regresan a su trabajo y a sus actividades habituales en cuestión de meses; algunas incluso vuelven a practicar deportes, como el golf. Su propia recuperación dependerá de su estado de salud, su condición física y la respuesta de su cadera al tratamiento.

Qué puede salir mal

La mayoría de los pacientes evolucionan bien, pero en ocasiones pueden surgir problemas. Su cirujano y el equipo lo vigilarán de cerca para detectar cualquier anomalía a tiempo.

En algunos casos, puede producirse una fisura en el hueso alrededor de las nuevas piezas, ya sea durante la operación o posteriormente. Es posible que sienta un dolor agudo y repentino en la cadera, el muslo o la ingle; o un dolor que aparece tras una caída o golpe. Si nota este tipo de dolor nuevo, comuníquese de inmediato con la clínica. Si el dolor es intenso, acuda a urgencias.

La nueva cadera también puede salirse de su cavidad; esto se denomina dislocación. Generalmente se percibe como un dolor fuerte y súbito; la pierna puede parecer más corta o adoptar una posición anormal, impidiendo soportar peso. Si esto ocurre, vaya a urgencias. En algunos casos la cadera se siente floja o inestable sin llegar a dislocarse por completo; mencione cualquier sensación de movilidad o inestabilidad de la cadera en su próxima consulta.

Las infecciones alrededor de la nueva articulación son poco frecuentes, pero requieren tratamiento rápido. Esté atento a un dolor profundo y palpitante que no ceda con analgésicos comunes, enrojecimiento que se extienda desde la herida, hinchazón progresiva o secreción de líquido desde el corte. También podría sentir fiebre o malestar general. Si observa estos signos, llame a la clínica ese mismo día; si se siente muy mal, acuda a urgencias.

Con el paso de los años, las superficies artificiales pueden desgastarse lentamente, y la unión entre las nuevas piezas y el hueso puede debilitarse. Esto suele manifestarse como un dolor sordo en la ingle o el muslo, que reaparece tras años de alivio; a veces se siente al ponerse de pie o al iniciar la marcha. Comente esto en su próxima revisión, ya que podría ser necesario realizar radiografías para evaluar la cadera.

¿Cuándo deben llamarnos?

La mayoría de los problemas presentan señales de alerta que se pueden detectar a tiempo. Llámenos si tienen fiebre o escalofríos, si la piel alrededor de la herida se vuelve más roja o comienza a exudar líquido, o si el dolor sigue empeorando en lugar de mejorar. Llámenos si una de las pantorrillas se hincha o se vuelve sensible al tacto; acudan a urgencias si experimentan dificultad para respirar. Acudan a urgencias si de repente sienten un dolor intenso en la cadera, o si la pierna se siente entumecida, parece más corta o está en una posición anormal, y no pueden moverla ni apoyar peso en ella.
