Adverse reaction to metal debris

What you're feeling

Metal debris from a worn hip implant can irritate the tissue around it. Some people feel a deep ache in the groin, thigh or buttock. The pain often worsens with walking, stairs or standing for long periods, and may flare at night or when you first get out of bed. Tasks like getting out of a low chair, putting on socks and shoes, or walking to the letterbox can become harder than they used to be.

Not everyone has pain. Some people notice nothing at all. Around half of people with certain metal-on-metal hips develop a soft fluid-filled swelling near the joint, called a pseudotumour, and many of these cause no symptoms. This is why we don't rely on how you feel alone. Your surgeon may arrange blood tests to measure cobalt and chromium levels, along with a special type of MRI scan that can see through the metal.

The swelling or irritation can sometimes affect the muscles and tissues holding the hip steady, which may make the joint feel unstable or give way. Skin discolouration over the hip, along with pain and poor function, can be a sign that the implant hardware is wearing out.

If your implant does need revising, most people find their metal levels drop back to very low levels afterwards, and pain relief is often substantial.

What's actually happening

A hip replacement is meant to let the joint move smoothly, like a well-oiled hinge. But metal parts rubbing on metal, or even metal parts joined to other metal parts, can slowly shed tiny particles. Think of two metal surfaces grinding together: over years, they throw off dust. That dust is metal debris.

Your body treats this debris as something foreign. The immune system, your built-in defence force, responds to it. That response can irritate and damage the soft tissues around the implant, including muscle, tendon and the joint lining. In some people this causes pain or a feeling that the hip is unsteady. In others it forms a soft fluid-filled swelling near the joint, which is what a pseudotumour is: not a cancer, but a pocket of inflamed tissue and fluid.

Two things drive this process. The first is wear: the metal surfaces slowly grinding each other down. The second is corrosion, a chemical reaction where tiny amounts of metal dissolve off the implant surfaces, especially where different metal parts connect. Wear and corrosion often happen together, each making the other worse. The debris particles from metal implants are extremely small, far smaller than the particles other implant materials produce, and that small size lets them work their way deep into the surrounding tissue.

The result is what your symptoms come from. The irritated tissue aches, swells and sometimes weakens the muscles that hold the hip steady. Blood tests can pick up raised cobalt and chromium levels, a sign the implant is shedding metal. Because some people feel nothing at all, your surgeon doesn't rely on symptoms alone to decide what's going on.

What we can do about it

Dr Kieran Hirpara, an orthopaedic surgeon at Mater Private Hospital Rockhampton, treats adverse reaction to metal debris by matching the response to what your tests and symptoms show. Patients are generally referred to our clinic by their GP; if a physiotherapist has suggested you see us, you will still need a referral from your GP in order to be eligible for the Medicare rebate. At your first visit we take a history, examine your hip, and arrange imaging and blood tests where needed.

Because some people with this condition feel nothing at all, we don't rely on symptoms alone. We combine several checks: your cobalt and chromium blood levels, a special MRI scan that can see through metal, and X-rays. No single test decides things on its own. If the scans show early wear changes in the bone near the implant, we keep a close eye on you with repeat imaging, because picking up the reaction early matters.

For people with mild findings and no damage to the bone, careful follow-up may be all that's needed for now. We will see you regularly and repeat the blood tests and scans to check whether things are stable or getting worse. If your levels rise or the swelling grows, we act on that rather than wait.

When the reaction has damaged tissue or bone, or your metal levels are clearly raised, surgery is usually the next step. This is called a revision, which means replacing some or all of the worn implant parts. Revising the implant removes the source of the debris. Most people find their metal levels drop back to very low levels afterwards. Before any revision, we also check thoroughly for infection near the implant, because an infection can look similar on tests and needs different treatment.

One thing worth knowing: if surgery is done for this condition, damaged tissue around the implant can slow early healing. A portion of people who have revision surgery for this problem need further procedures later. We weigh this up with you carefully, and the decision is one you make together with us, based on your symptoms, your scans and what matters most to you.

What to expect

The outlook depends a lot on what your tests show and whether you have symptoms. Many people with small swellings near the implant feel nothing, and those often stay that way. Most small fluid-filled swellings seen on scans in people without symptoms shrink over time rather than grow, and most show no change at all over several years. A smaller number slowly enlarge, which is why we keep checking rather than assume.

If you do have pain or swelling, it rarely settles on its own while the implant keeps shedding metal. The irritation tends to persist or slowly worsen, because the cause is the implant surface itself, not a strain or injury that time will heal. Leaving it alone does not usually lead to steady improvement. The main reason to keep an eye on it is that catching changes early gives you more options before tissue around the hip is badly affected.

When surgery is needed, removing the worn parts removes the source of the debris. Most people find their metal levels drop back to very low levels afterwards, and pain relief is often substantial. It is honest to say that recovery from this particular operation can be slower than from a first hip replacement, because damaged tissue around the implant can slow early healing. Some people need further procedures later, and we will talk through that risk with you before any decision is made.

Two reassurances worth knowing. Nerve problems sometimes linked to metal hips, such as changes in hearing, vision or sensation, are no more common than with other types of hip replacement. And this condition does not appear to stop a revised implant from bonding securely to your bone.

What we ask of you is simple: come to your follow-up appointments, have the blood tests and scans we arrange, and tell us if your pain changes or you notice new swelling. That way, if anything shifts, we act on it early.

When to see someone

See your GP if you have a deep ache near the hip that will not settle, or if you notice a new lump or swelling in the groin or near the joint. Ask for a specialist review if the hip feels unstable or gives way, or if the skin over the hip changes colour while the hip is painful and not working well. Tell your GP if you have already been told you have a pseudotumour and it grows, or if your pain changes. Some people with this condition feel nothing at all, so a check-up matters even when symptoms are mild. If you have already had blood tests showing raised cobalt or chromium levels, keep to the follow-up plan you were given, because rising levels can be a sign the implant is wearing faster.

4. Media and formats; technical modifications allowed. b. Other rights.

a. retain the following if it is supplied by the Licensor with the Licensed Material:

i. identification of the creator(s) of the Licensed Material and any others designated to receive attribution, in any reasonable manner requested by the Licensor (including by pseudonym if designated);

ii. a copyright notice;

iii. a notice that refers to this Public License;

iv. a notice that refers to the disclaimer of warranties;

v. a URI or hyperlink to the Licensed Material to the extent reasonably practicable;

b. indicate if You modified the Licensed Material and retain an indication of any previous modifications; and

b. if You include all or a substantial portion of the database contents in a database in which You have Sui Generis Database Rights, then the database in which You have Sui Generis Database Rights (but not its individual contents) is Adapted Material; and

c. You must comply with the conditions in Section 3(a) if You Share all or a substantial portion of the contents of the database.

For the avoidance of doubt, this Section 4 supplements and does not replace Your obligations under this Public License where the Licensed Rights include other Copyright and Similar Rights.
