Why this operation has been suggested¶

Dr Kieran Hirpara, an upper-limb surgeon at Mater Private Hospital Rockhampton, starts with the least invasive options that suit your condition. You reach our clinic by GP or physiotherapist referral. A clinic assessment establishes the diagnosis. For long-standing problems we usually try non-operative care first. We consider surgery when that has not given enough improvement.

Most ganglions recur after aspiration. Open surgical excision offers a significantly lower chance of recurrence compared with aspiration. Surgical intervention for wrist ganglions has approximately a 10% recurrence rate. This operation aims to reduce pain and improve function. About 40% of wrist ganglion lesions decrease over the first 6 years after evaluation by a hand surgeon. If your symptoms persist, surgery may be the right next step.

Before the operation¶

You must fast for six hours before your procedure. Please stop taking blood-thinning medications as directed by your surgeon. Arrange for a responsible adult to drive you home, as you cannot drive yourself after anaesthesia. Bring a complete list of all current medications and supplements. You may need an X-ray or MRI to confirm the cyst’s location, or blood tests and an anaesthetic review to ensure you are safe for surgery. Wear loose, comfortable clothing on the day. We will provide specific instructions on medication timing and fasting windows closer to your date.

On the day¶

You present to the hospital's surgical admissions unit, where you are checked in and prepared for theatre. You meet the anaesthetist to discuss your care plan. This operation is done under general anaesthetic. You will be fully asleep for the operation. Some patients may also have a regional nerve block for post-operative pain relief — the anaesthetist decides on the day based on your individual circumstances.

You are then taken into the operating theatre, where the operation is performed. You wake up in the recovery area, where nurses monitor you while the anaesthetic wears off. Once you are stable you either go to the ward or go home, depending on the procedure and your recovery.

What the operation involves¶

Your surgeon chooses the approach based on where your ganglion sits. For cysts on the back of your wrist, we use an arthroscopic technique. This means we make two or three very small keyhole cuts. We insert a tiny camera and thin instruments through these openings. This allows us to see inside the joint clearly while keeping the cuts minimal.

For cysts on the palm side of your wrist, we use an open surgical excision. This involves a single cut along the palm side of your wrist. Through this incision, your surgeon carefully accesses the cyst and removes it completely. This method is often preferred for volar ganglia because it allows direct visualization of the surrounding structures.

During the procedure, your surgeon removes the ganglion cyst and its root to prevent it from filling up again. If your ganglion is located near major blood vessels, your surgeon takes extra care to protect them. After the cyst is removed, the small cuts are closed with stitches or glue. A sterile dressing is applied to keep the area clean.

We do not routinely send the removed tissue for pathological examination if the diagnosis was clear before surgery. This step is not necessary for quality of care in typical cases. The entire process is focused on removing the fluid-filled sac that is causing your discomfort.

After the operation¶

You will wake up in the recovery ward with your wrist in a dressing and a sling. We provide general pain control to keep you comfortable. This is usually a day case, so you can expect to go home the same day, although occasionally patients stay overnight. Please ensure someone stays with you for the first 24 hours. You must not drive while wearing a splint. Most patients return to driving within two to three weeks for this wrist procedure, once any post-op splint is off and they can hold the wheel comfortably. Recovery¶

You will notice swelling and stiffness in your wrist as it heals. This is normal. Your surgeon will guide you on managing this discomfort. Most patients find that pain decreases significantly within six weeks as movement returns. You may need to keep your wrist elevated to help reduce swelling.

We use hand therapy with Ruby Doolan at Extend Rehabilitation to support your recovery. Your physiotherapist will teach you gentle exercises to restore strength and flexibility. You will start with simple movements and gradually increase activity as your wrist allows. Avoid forceful wrist extension, especially if your job requires it, as this can cause residual pain.

You can return to driving once your surgeon clears you to do so. This typically happens within two to three weeks, once any post-op splint is off and you can hold the wheel comfortably. Do not drive while wearing a splint. You will also be able to resume light daily tasks as comfort allows. Your timeline may differ; your surgeon and physio will guide you through each step.

What can go wrong¶

Most patients do well, but problems can occasionally happen. Your surgeon and the team monitor you closely to spot any issue early.

Pain and stiffness You might feel deep, throbbing pain that does not ease with simple painkillers. This is more likely if your job requires forceful wrist extension. You may also notice stiffness or a clicking feeling in your wrist. If pain persists or worsens, call the clinic.

Swelling and recurrence The ganglion may return, causing sudden swelling and tenderness. You might see the lump reappear at the same spot. This can happen with both open and arthroscopic surgery. If the swelling returns or causes discomfort, bring it up at your next review.

Nerve and artery injury Rarely, surgery can affect nearby nerves or blood vessels. You might notice numbness, tingling, or weakness in your hand or fingers. In very rare cases involving sclerotherapy (a needle injection), there is a risk of injury to the radial artery. If you feel unusual numbness or see changes in skin colour, go to the emergency department immediately.

Scarring and infection Surgery leaves scars. You might notice redness, warmth, or drainage from the wound site. If you see redness spreading out from the wound or feel feverish, call the clinic right away.

General risks Some patients remain dissatisfied with the outcome despite symptom relief. You might feel that function has not improved as much as hoped. Discuss any concerns about your recovery progress with your surgeon.

When to call us¶

Call us if you have fever, increasing wound redness, or discharge. Go to emergency if you feel sudden severe pain, calf swelling, or shortness of breath. Contact us immediately for loss of sensation or inability to move your limb. We want to ensure your recovery stays on track. Please do not wait if these symptoms appear.
