Why this operation has been suggested¶

This page reflects how Dr Kieran Hirpara, an upper-limb surgeon at Mater Private Hospital Rockhampton, approaches this in our clinic. We recommend this procedure when non-operative care has not given enough improvement. You reach our clinic by GP or physiotherapist referral. A clinic assessment establishes the diagnosis. You have a ganglion cyst in your flexor tendon sheath. This is a fluid-filled lump near your tendons. Most ganglions recur after aspiration. Surgical intervention for wrist ganglions has about a 10% recurrence rate. Open surgical excision offers a significantly lower chance of recurrence compared with aspiration. We perform this as an open approach with a single conventional incision over the operative site. The main benefit is to relieve pain and restore function.

Before the operation¶

We ask you to fast for six hours before your surgery. Please stop taking blood-thinning medicines as your surgeon will advise. Arrange a lift home, as you cannot drive yourself. Bring a list of all current medications and wear comfortable, loose clothing. Your surgeon may order X-rays or an MRI to see the cyst clearly. Blood tests and an anaesthetic review help ensure your safety. These steps prepare your body for the procedure. We perform this as an open operation with a single incision. On the day¶

This operation is done under general anaesthetic. You will be fully asleep for the operation. Some patients may also have a regional nerve block for post-operative pain relief — the anaesthetist decides on the day based on your individual circumstances.

You will arrive at the hospital and meet your surgeon before going to the operating theatre. We use an open approach with a single conventional incision over the operative site. This allows direct access to the ganglion for removal. Your surgeon will guide you through each step. After the procedure, you will wake up in recovery. Our team will monitor you closely as the anaesthetic wears off. You can expect to rest in a comfortable bed while we check your pain levels and circulation. What the operation involves¶

Your surgeon performs this procedure using an open approach. This means a single conventional cut is made directly over the area where the ganglion cyst is located. This method allows your surgeon to see the structures clearly and remove the cyst completely.

Inside, your surgeon carefully identifies the fluid-filled sac that has formed within or near your tendon sheath. The surgeon then separates the cyst from the surrounding healthy tissue, including the tendons and nerves, to ensure they are not damaged. Once the cyst is fully isolated, it is removed from your body. This process addresses the source of the swelling and any pressure it may have been placing on nearby structures.

After the cyst is removed, your surgeon closes the cut with stitches. The area is then dressed with a bandage to protect the site as it begins to heal. This straightforward technique allows for direct removal of the ganglion while minimizing the need for multiple small incisions or specialized equipment.

After the operation¶

You will wake up in the recovery ward. Your surgeon will manage your pain and care for your wound. We apply a soft dressing to protect the area; no splint or plaster is needed. You should have someone stay with you for the first 24 hours. This is usually a day case, so you can expect to go home the same day, although occasionally patients stay overnight. Most patients return to driving within one to two weeks, once the wound is comfortable and you can grip and turn the wheel without protecting the operated hand. Recovery¶

You will notice some swelling and stiffness in your hand and wrist during the first few days. This is a normal part of healing. We keep the area elevated and use ice packs to help ease the discomfort. There is no splint; the soft dressing protects the incision while you rest. Keep the dressing clean and dry as we advise.

Once your surgeon clears you to drive, you can return to the road. This usually happens when the wound is comfortable and you can grip the steering wheel without pain or hesitation. Most patients find they are ready to drive within one to two weeks, depending on how quickly the swelling settles.

These activities help prevent stiffness and support a smooth return to daily tasks. You can gradually resume light household chores as your grip strength returns. Avoid heavy lifting or forceful wrist movements until your surgeon gives the all-clear.

What can go wrong¶

Most patients do well, but problems can occasionally happen. Your surgeon and the team monitor you closely to spot any issue early.

The most common issue is the ganglion returning. This means the fluid-filled sac grows back in the same spot. You might notice a new lump or swelling near your original incision. It may feel tender or cause mild discomfort. If you see this happening, let us know at your next review. We can discuss whether further treatment is needed.

Scar tissue is another possibility. Surgery leaves a scar, which is normal. However, some people develop thick or raised scars. You might see redness or feel tightness in the skin around the wound. In rare cases, the scar can become itchy or painful. If the scar bothers you or looks infected, contact our clinic. We can advise on scar care options.

Infection is a risk with any surgery. You might notice increased pain, warmth, or redness spreading from the wound. There could be pus or unusual drainage. You might also develop a fever. If you see these signs, do not wait. Call the clinic immediately or go to the emergency department. Early treatment with antibiotics usually resolves this quickly.

Nerve irritation can occur near the surgical site. You might feel tingling, numbness, or a burning sensation in your hand or fingers. This can happen if the nerves are stretched or irritated during the procedure. Most cases improve over time as healing occurs. If the sensation is severe, persistent, or worsening, tell us right away. We will check your nerve function and adjust your care plan.

Blood vessel injury is rare but serious. You might notice sudden, significant swelling or bruising in your hand or wrist. The area might feel cold or look pale. This requires immediate medical attention. Go to the emergency department if this happens. Prompt care helps prevent long-term damage to blood flow in your hand.

When to call us¶

Call us if you notice fever, increasing wound redness, or discharge. Seek emergency care for sudden severe pain, calf swelling, or shortness of breath. Contact us immediately if you experience loss of sensation or inability to move your hand. These signs need urgent assessment to ensure your recovery stays on track.
